摘 要:目的比较西藏高海拔地区和成都平原地区的藏族和汉族患者龈下微生物群落结构,初步探讨高海拔、民族和牙周炎症对龈下微生物群落结构的影响。方法纳入长期在西藏的藏族牙周炎患者38例和汉族牙周炎患者30例,长期在成都的汉族牙周炎患者31例,并与健康组进行对比,使用Illumina MiSeq测序平台进行龈下菌斑16S rRNA基因测序,对比分析各组之间的微生物群落结构差异。结果牙周炎组较牙周健康组物种组成高。对于汉族牙周炎患者,海拔越高则龈下微生物alpha多样性越低(P=0.009),beta多样性差异也有统计学意义(P=0.003),但对汉族牙周健康者,海拔对于alpha多样性和beta多样性均无显著影响(P=0.071)。牙周健康组较牙周炎组龈下微生物群落alpha多样性高(P<0.001),二者beta多样性分析差异也有统计学意义(P=0.001)。藏族和汉族的龈下微生物beta多样性相似(P=0.431)。平原汉族牙周炎组的优势菌群有牙龈卟啉单胞菌、拟杆菌。高原汉族牙周炎组的优势菌群是普氏菌。平原汉族牙周炎组与高原汉族牙周炎组相比,卟啉单胞菌丰度显著升高。平原健康组的弯曲杆菌、梭菌属丰度较高原健康组高,而高原健康组的艾肯菌属丰度较高。在高原牙周炎中藏族与汉族人群的牙周常见菌群无显著差异。结论高海拔和牙周健康状况对龈下微生物群落结构有一定影响,相对而言民族之间差异较小。Objective To compare the subgingival microbiota of Tibetan and Han patients in high altitude area of Tibet and Chengdu,and explore the impacts of high altitude,ethnicity and periodontitis on subgingival microbiota.Methods A total of 38 Tibetan periodontitis patients and 30 Han periodontitis patients in Tibet for a long time as well as 31 Han periodontitis patients in Chengdu for a long time were enrolled,and compared with the healthy controls.Subgingival plaques were analyzed using Illumina MiSeq sequencing platform.The 16S rRNA gene high-throughput sequencing was used to compare and analyze the differences in microbiota among the groups.Results The subgingival microbial species composition in periodontitis patients was higher than that in healthy controls.In Han periodontitis patients,the higher the altitude,the lower the alpha diversity of subgingival microbiota(P=0.009),and the beta diversity was also significantly different(P=0.003);but for Han healthy controls,the altitude had no obvious impact on the alpha diversity and beta diversity(P=0.071).The alpha diversity of subgingival microbials was higher in healthy controls than in periodontitis patients(P<0.001),and the beta diversity was also statistically different between the two groups(P=0.001).The beta diversity was similar between the Tibetans and Hans(P=0.431).Porphyromonas gingivalis and Bacteroidetes were the dominant bacteria in the periodontitis group of plain Hans.The dominant bacteria in the periodontitis group of plateau Hans was Prevotella.Compared with the plateau Han periodontitis group,Porphyromonas was significantly higher in the plain Han periodontitis group.Campylobacter and Clostridium were higher in plain healthy controls than in plateau healthy ones,while Eikenella was higher in plateau healthy individuals.There was no significant difference in common periodontal microbials between Tibetans and Hans in plateau periodontitis groups.Conclusion High altitude and periodontal health have impact on the structure of periodontal microbiota;there
