基于人工智能探究不同年资医师各肠段有效退镜时间对结肠镜检查质量的影响

摘　　要：目的通过基于人工智能(artificial intelligence,AI)的结肠镜有效退镜时间计算系统,探究不同年资医师各肠段的有效退镜时间的阈值。方法回顾性收集在武汉大学人民医院就诊的760例患者肠镜视频,和对应的患者人口学信息、肠镜检查以及病理报告,使用本项目组前期构建的计算系统智能化分析不同年资医师各肠段有效退镜时间,并研究其对结肠镜质量的影响。结果以人工测量有效退镜时间为金标准,AI辅助计算有效退镜时间的准确度为93.00%。不同年资医师各肠段有效退镜时间阈值分别为:(1)高年资医师:右半结肠段89.92 s,横结肠段92.73 s,左半结肠段150.06 s;(2)低年资医师:右半结肠段82.51 s,横结肠段131.57 s,左半结肠段180.21 s。当低年资医师右半结肠、横结肠以及左半结肠退镜时间分别大于82.51 s、137.57 s和180.21 s时,可显著提高低年资医师的腺瘤检出率(P<0.001)。结论建议低年资医师右半结肠、横结肠及左半结肠有效退镜时间至少为82.51 s、131.57 s和180.21 s,高年资医师各肠段有效退镜时间至少为89.92 s、92.73 s和150.06 s,结肠镜检查质量较优。Objective To explore the threshold of effective colonoscopy withdrawal time for each colon segment of different seniority endoscopists by artificial intelligence(AI)-based computing system.Methods This study retrospectively collected 760 colonoscopy videos from Renmin Hospital of Wuhan University,and the corresponding demographic information,colonoscopy reports and pathological reports of patients.Using the computational system constructed by our project team in the early stage,intelligently analyzed the effective withdrawal time of different intestinal segments by physicians with different qualifications.Its impact on the quality of colonoscopy was investigated.Results The accuracy of AI-assisted calculation was 93.00%with the gold standard of manual measurement of effective withdrawal time.The effective withdrawal time threshold of each segment of endoscopists with different seniorities:(1)senior endoscopists:right colon segment was 89.92 s,transverse colon segment was 92.73 s and left colon segment was 150.06 s.(2)junior physicians:right colon segment was 82.51 s,transverse colon segment was 131.57 s and left colon segment:180.21 s.When the withdrawal time of right colon,transverse colon and left colon was more than 82.51 s,137.57 s and 180.21 s,respectively,ADR of junior endoscopists could be significantly improved(P<0.001).Conclusion It is recommended that the effective withdrawal time of right colon,transverse colon and left colon for junior physicians should be at least 82.51 s,131.57 s and 180.21 s,and that for senior physicians,the effective withdrawal time of each intestinal segment should be at least 89.92 s,92.73 s and 150.06 s.The quality of colonoscopy is better.
