摘　　要：目的比较分析相关数据,从出院患者人次、患者住院天数、收治病种、CMI值等方面对医院专科、亚专科设置与效果进行评价,以进一步优化医院专科设置。方法选取某三甲医院7个临床重点学科2018—2020年住院患者信息,运用描述性统计学、t检验和卡方检验等方法,比较7个学科、30个亚专科医疗服务能力提升情况。结果专科总出院人次增加,平均住院日均缩短,6个专科CMI值增加,1个专科CMI值减少。亚专科出院人次、平均住院日均向好,CMI值增幅无显著性差异,亚专科收治病种结构不断优化。结论亚专科建设可促进专科医疗服务能力的提升,同一专科不同的亚专科建设效果不一,亚专科的出院人次、平均住院日及CMI值相互影响。建议结合专科发展方向科学设置亚专科,进一步完善亚专科评价体系,加强亚专科建设相关配套支持措施。Objective To continually optimize the subspecialty setting by comparing and analyzing relevant data,evaluating the effects of hospital subspecialty setting from discharged patients,disease categories and CMI value.Methods Data of inpatients of 7 key clinical disciplines in a tertiary hospital from 2018 to 2020 were selected.Descriptive statistics,t-test and chi-square test were used to compare the improvement of medical service capacity of 7 disciplines and 30 subspecialties.Results The total number of specialty discharges increased,the average length of stay shortened,the CMI value of 6 specialties increased,and the CMI value of 1 specialty decreased.The number of subspecialty discharges and the average length of stay of the subspecialty were improved,and there was no significant difference in the increase of CMI value.The structure of diseases admitted to the subspecialty was continuously optimized.Conclusion Subspecialty construction can promote the improvement of specialized medical service capacity.Different subspecialty construction in the same specialty has different effects,and the discharge number,average length of stay and CMI value of subspecialties affect each other.It is suggested to scientifically set up subspecialties according to the development direction of specialties,further improve the evaluation system of subspecialties,and strengthen the supporting measures related to the construction of subspecialties.
