慢性肝病患者血清基质金属蛋白酶-3检测的临床价值研究

摘　　要：目的探讨血清基质金属蛋白酶-3(MMP-3)在慢性肝病患者中的表达水平及临床价值。方法收集2021年10月—2022年10月本院住院慢性肝病患者244例,并以健康体检者90例作为正常对照,采用乳胶增强免疫比浊法检测血清中MMP-3浓度,并对检测结果进行统计学分析。结果慢性肝病患者血清MMP-3水平较正常对照组显著上调(P均<0.05);随着病情进展,血清MMP-3水平逐渐升高,其中肝癌组显著高于慢性肝炎组(P<0.05);不同疾病组中男性患者血清MMP-3水平均显著高于女性,差异均有统计学意义(P均<0.05),但与年龄无显著相关性(P均>0.05);肝硬化患者血清MMP-3水平随CTP评分的升高而逐渐升高,特别是B、C级显著高于A级,差异均有统计学意义(P均<0.05);肝癌患者血清MMP-3水平与临床病理特征均无显著相关性(P均>0.05)。结论血清MMP-3在慢性肝脏疾病中表达水平上调,但其单独应用于慢性肝病鉴别诊断的价值可能有限。Objective This paper aims to investigate the level of serum matrix metalloproteinase-3(MMP-3)in patients with chronic liver disease and its clinical value.Methods A total of 244 patients with chronic liver disease admitted to our hospital from October 2021 to October 2022 were collected,and 90 healthy people were taken as the controls.The concentration of MMP-3 in serum was detected by latex enhanced immune turbidimetry,and the results were statistically analyzed.Results The level of serum MMP-3 in patients with chronic liver disease was significantly higher than that in the controls(P<0.05).With the progression of the disease,the level of serum MMP-3 gradually increased,and the level of liver cancer group was significantly higher than that of chronic hepatitis group,with the differences statistically significant(P<0.05).In different disease groups,the serum MMP-3 level of male patients was significantly higher than that of female patients(P<0.05),but there was no significant correlation with age(P>0.05).The serum MMP-3 level in patients with liver cirrhosis gradually increased with the increase of Child-Turcotte-Pugh(CTP)score,and the levels of grade B and C groups were significantly higher than that of grade A group,with the differences statistically significant(P<0.05).There was no significant correlation between serum MMP-3 level and clinicopathological features in patients with liver cancer(P>0.05).Conclusion The expression of serum MMP-3 is generally up-regulated in chronic liver disease,but its value in differential diagnosis of chronic liver disease may be limited.
