摘　　要：目的评估中性粒细胞与淋巴细胞比值(NLR)对胰腺癌非手术患者预后的影响。方法回顾性选取2016年1月1日—2021年9月30日宁波市医疗中心李惠利医院首次被确诊为胰腺癌并行非手术治疗患者125例,通过电子病历收集其临床资料及相关检测指标,并通过随访获取患者预后信息。结果6个月随访总生存期(OS)的受试者工作特征曲线(ROC)曲线下面积,NLR最高为0.756(P<0.001)。高NLR组患者与转移性胰腺癌相关(P=0.038)。Kaplan-Meier生存曲线分析发现,高NLR与更差的OS相关(P<0.001)。多因素Cox回归分析发现,高NLR、转移性胰腺癌、化疗或放化疗、高C-反应蛋白/白蛋白(CRP/Alb)比值、高CA19-9与OS独立相关(P<0.05)。结论在诊断时高NLR可能与胰腺癌非手术患者预后不良相关。Objective This paper aims to evaluate the effect of neutrophil lymphocyte ratio(NLR)on prognosis of non-surgical patients with pancreatic cancer.Methods A total of 125 patients who were diagnosed with pancreatic cancer for the first time and received non-surgical treatment in Ningbo Medical Center Lihuili Hospital from January 1,2016 to September 30,2021 were retrospectively selected.Clinical data and related detection indicators were collected through electronic medical records,and prognostics of patients were obtained through follow-up.Results The area under the ROC curve of Overall Survival(OS)was followed up for 6 months,and NLR was 0.756,which was the highest(P<0.001).Patients with high NLR were associated with metastatic pancreatic cancer(P=0.038).It was found that higher NLR was associated with worse OS in Kaplan-Meier survival curve analysis(P<0.001).Multivariate Cox regression analysis showed that high NLR,metastatic pancreatic cancer,chemotherapy or chemo-radiotherapy,high C-reactive protein/albumin(CRP/Alb)ratio,high CA19-9 were independently associated with OS(P<0.05).Conclusion High NLR at diagnosis may be associated with poor prognosis of non-surgical patients with pancreatic cancer.
