不同开窗术式对上颌埋伏中切牙牵引效果影响的研究

摘　　要：目的 探讨不同开窗术式对上颌埋伏中切牙正畸牵引效果的影响。方法 选择2015年9月-2022年8月在某部队医院口腔科就诊且采用外科开窗-正畸牵引方案治疗上颌埋伏中切牙患者34例作为研究对象,外科开窗手术分别采用开放式开窗(14例)和闭合式开窗(20例),正畸结束后通过对埋伏牙牙根长度、牙槽骨覆盖情况和软组织美学评分等三个方面进行评价,研究不同外科开窗术式对埋伏中切牙正畸牵引效果的影响。结果 34颗上颌埋伏中切牙均顺利进入牙列,排齐且正常行使功能。闭合组与开放组的埋伏牙牙根长度、牙槽骨覆盖情况均无统计学差异(P>0.05);闭合组软组织美学评分高于开放组(P<0.05)。结论 闭合式开窗手术在前牙区的牙周美学方面优于开放式开窗手术,在牙根发育情况及牙槽骨覆盖方面无明显差异。Objective To investigate the influence of different fenestration techniques on orthodontic traction of maxillary impacted central incisors.Methods Thirty-four cases of patients with maxillary impacted central incisors who were treated with surgical fenestration-orthodontic traction scheme in the stomatology department of a military hospital from 2015 to 2022 were enrolled as study subjects.The fenestration including open fenestration technique(n=14)and closed fenestration technique(n=20).After treatment,the effects of different fenestration techniques on the orthodontic traction of impacted central incisors were evaluated from three aspects:root length,alveolar bone coverage and soft tissue aesthetic score.Results All the 34 cases of maxillary impacted central incisors entered the dentition smoothly,aligned and performed their functions.There were no significant difference in root length and alveolar bone coverage between the two groups(P>0.05);the soft tissue aesthetic score of the closed group was higher than that of open group(P<0.05).Conclusions Closed fenestration technique is superior to open fenestration technique in periodontal beauty of anterior teeth,but there is no significant difference in the development of tooth roots and alveolar bone coverage.
