摘　　要：目的分析血清白蛋白、总蛋白水平对早期结肠癌患者内镜黏膜下剥离术后复发的影响。方法纳入郑州市中心医院2019年8月至2021年8月治疗的早期结肠癌患者作为研究对象,所有患者均接受内镜黏膜下剥离术治疗,术前记录患者肿瘤分期、肿瘤部位等资料;术后3 d,测定血清白蛋白、总蛋白水平,术后对患者进行跟踪随访,观察肿瘤复发情况,分析血清白蛋白、总蛋白水平对早期结肠癌患者内镜黏膜下剥离术后复发的影响。结果本研究共纳入123例早期结肠癌患者进行研究,中途失访5例,最终纳入118例患者。随访至2022年8月,118例早期结肠癌患者有20例复发,复发率为16.95%;复发组血清白蛋白、总蛋白水平低于未复发组,血清C反应蛋白水平高于未复发组,差异有统计学意义(P<0.05)。经logistic回归分析显示,年龄、肿瘤分期、血清癌胚抗原、白蛋白、总蛋白及C反应蛋白均是早期结肠癌患者内镜黏膜下剥离术后复发的影响因素(P<0.05)。结论血清白蛋白、总蛋白水平与早期结肠癌患者内镜黏膜下剥离术后复发密切相关,且血清白蛋白、总蛋白是患者术后肿瘤复发的影响因素。Objective To analyze the effect of serum albumin and total protein levels on recurrence of patients with early colon cancer after endoscopic submucosal dissection.Methods Early colon cancer patients treated in Zhengzhou Central Hospital from August 2019 to August 2021 were included as the study subjects.All patients were treated with endoscopic submucosal dissection.The data of tumor stage and tumor location were recorded before surgery.Three days after surgery,serum albumin and total protein levels were measured.Patients were followed up to observe tumor recurrence and analyze the impact of serum albumin and total protein levels on postoperative recurrence in early colon cancer patients undergoing endoscopic submucosal dissection.Results A total of 123 patients with early colon cancer were included in this study,5 patients were lost during the study,and 118 patients were finally included.Follow up to August 2022,20 of 118 patients with early colon cancer recurred,with a recurrence rate of 16.95%.The levels of serum albumin and total protein in the recurrent group were lower than those in the non recurrent group,and the levels of serum C-reactive protein were higher than those in the non recurrent group(P<0.05).According to point two column correlation analysis,the recurrence rate of early colon cancer patients after endoscopic submucosal dissection was negatively correlated with serum albumin and total protein levels,and positively correlated with C-reactive protein levels(P<0.05).Logistic regression analysis showed that age,tumor stage,serum carcinoembryonic antigen,albumin,total protein and C-reactive protein were all influencing factors for postoperative recurrence in early colon cancer patients undergoing endoscopic submucosal dissection(P<0.05).Conclusion Serum albumin and total protein levels is closely related to the recurrence of early colon cancer patients after endoscopic submucosal dissection,and serum albumin and total protein is an influential factor for tumor recurrence.
