摘 要:目的探讨分化型甲状腺癌(DTC)组织中Ki-67、半乳糖凝集素-3(Gal-3)的表达及其与术后淋巴结转移的关系,以期为临床改善预后提供数据支持。方法选取许昌市中心医院2020年4月至2022年8月收治的拟行手术治疗的256例DTC患者作为研究对象,对比癌组织与癌旁组织、不同病理特征、不同术后淋巴结转移者Ki-67、Gal-3表达,分析癌组织Ki-67、Gal-3表达对术后淋巴结转移的预测价值。结果DTC患者癌组织中Gal-3、Ki-67阳性表达率高于癌旁组织(P<0.05);Ⅲ~Ⅳ期、有淋巴结转移、T_(3)+T_(4)浸润深度的DTC患者癌组织中Gal-3、Ki-67阳性表达率高于Ⅰ~Ⅱ期、无淋巴结转移、T_(1)+T_(2)浸润深度患者(P<0.05);术后发生淋巴结转移DTC患者癌组织中Gal-3、Ki-67阳性表达率高于未发生淋巴结转移患者(P<0.05);受试者工作特征(ROC)曲线表明,癌组织中Ki-67、Gal-3表达联合预测DTC患者发生术后淋巴结转移的曲线下面积(AUC)是0.803,敏感度、特异度为80.39%、85.37%(P<0.05);癌组织中Gal-3、Ki-67阳性表达的DTC患者发生术后淋巴结转移的危险度为阴性表达者的4.442倍、3.131倍(P<0.05)。结论DTC癌组织中Gal-3、Ki-67多呈阳性表达,可反映病情程度,且联合应用时可有效预测术后淋巴结转移的发生风险,能为临床早期防治措施提供可参考依据。Objective To investigate the expression of Ki-67 and galectin-3(Gal-3)in differentiated thyroid carcinoma(DTC)and their relationship with postoperative lymph node metastasis,in order to provide reliable data support for clinical improvement of prognosis.Methods A total of 256 cases of DTC patients who were admitted to Xuchang Central Hospital from April 2020 to August 2022 for surgical treatment were selected as research objects,and the expressions of Ki-67 and Gal-3 in cancer tissues and adjacent tissues,different pathological features,and different postoperative lymph node metastases were compared.The prediction values of Ki-67 and Gal-3 expression in cancer tissue were analyzed for postoperative lymph node metastasis.Results The positive expression rates of Gal-3 and Ki-67 in cancer tissues of DTC patients were higher than those in paracancer tissues(P<0.05).The positive expression rates of Gal-3 and Ki-67 in cancer tissue of DTC patients in stageⅢtoⅣwith lymph node metastasis and T_(3)+T_(4)infiltration depth were higher than those in stageⅠtoⅡwith no lymph node metastasis and T_(1)+T_(2)infiltration depth(P<0.05).The positive expression rates of Gal-3 and Ki-67 in cancer tissues of DTC patients with postoperative lymph node metastasis were higher than those of patients without lymph node metastasis(P<0.05).Receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of Ki-67 and Gal-3 in cancer tissue combined to predict postoperative lymph node metastasis in DTC patients was 0.803,and the optimal sensitivity and specificity were 80.39%and 85.37%(P<0.05).The risk of postoperative lymph node metastasis in DTC patients with Gal-3 and Ki-67 positive expression in cancer tissues was 4.442 times and 3.131 times higher than that in patients with negative expression(P<0.05).Conclusion The positive expression of Gal-3 and Ki-67 in DTC cancer tissues can reflect the severity of the disease,and the combined application can effectively predict the risk of postoperative lymph node metasta
