摘 要:目的研究磁共振对比剂钆喷酸葡胺(Gd-DTPA)在肝脏脂肪分数(FF)定量测量的影响。方法选取2017年7月至2018年12月于本院行上腹部MRI检查的25例非酒精性脂肪性肝病(NAFLD)患者作为研究对象,在增强前后均进行mDixion序列MRI扫描,分别测量增强前后肝脏、脾脏、椎体及皮下脂肪的FF值和T_(2)^(*)值,同时计算增强前后肝脏/脾脏、肝脏/椎体、肝脏/皮下脂肪的FF比值和T_(2)^(*)比值。Bland-Altman图评估增强前后肝脏FF值的一致性,采用独立样本配对t检验评估增强前后FF值和T_(2)^(*)值差异。结果肝脏和脾脏增强前后FF值比较差异均无统计学意义,椎体和皮下脂肪增强后FF值均低于增强前(P<0.05),肝脏/脾脏、肝脏/椎体、肝脏/皮下脂肪FF比值比较差异均无统计学意义。肝脏、脾脏增强后的T_(2)^(*)值均低于增强前(P<0.05),而椎体和皮下脂肪增强前后的T_(2)^(*)值比较差异无统计学意义,肝脏/脾脏、肝脏/椎体、肝脏/皮下脂肪增强前后的T_(2)^(*)比值差异均无统计学意义。结论Gd-DTPA可增加肝脏T_(2)^(*)效应,T_(2)^(*)校正的mDixon技术能稳定、准确地测量肝脏脂肪定量,而椎体和皮下脂肪FF值被低估,可能是由于Gd-DTPA导致T1效应偏倚增大的影响。Objective To investigate the effect of gadopentetic acid contrast agents on hepatic fat quantification using multi-echo modified Dixon(mDixon)technique.Methods A total of 25 patients with non-alcoholic fatty liver disease(NAFLD)who underwent upper abdominal MRI examination in our hospital from July 2017 to December 2018 were selected as the study subjects,mDixion sequential MRI scans were performed before and after enhancement,and FF values and T_(2)^(*)values of liver,spleen,vertebra and subcutaneous fat were measured before and after enhancement,respectively.Meanwhile,FF and T_(2)^(*)rates of liver to spleen,spine and subcutaneous fat were calculated,respectively.Bland-Altman plots were used to assess the agreements between before and after enhancement FF values of hepatic,independent sample paired t test were performed to compare measurements.Results There were no statistically significant differences in FF values of liver and spleen before and after enhancement,while FF values of vertebral body and subcutaneous fat after enhancement were lower than before enhancement(P<0.05);there were no statistically significant differences in FF ratios of liver/spleen,liver/vertebral body,and liver/subcutaneous fat.The T_(2)^(*)values of liver and spleen after enhancement were lower than those before enhancement(P<0.05),while the T_(2)^(*)values of vertebral body and subcutaneous fat were not significantly different before and after enhancement,and there were no statistically significant differences in T_(2)^(*)ratios between liver/spleen,liver/vertebral body and liver/subcutaneous fat before and after enhancement.Conclusion Although under the impact of an decreased T_(2)^(*)in liver and spleen after Gd-DTPA administration,it can still stably and accurately obtain hepatic fat quantification using multi-echo mDixon technique with T_(2)^(*)correction,the underestimated FF values of spine and subcutaneous fat may due to an increase of the effect of T1 related bias post-contrast.
