摘　　要：目的探讨富血小板纤维蛋白(PRF)对下颌智齿拔除患者创面预后的影响。方法选取2020年10月至2021年10月赣州市皮肤病医院门诊收治的50例下颌智齿拔除患者作为研究对象,按随机数字表法分为PRF组与对照组,每组25例。对照组于智齿拔除术后进行常规缝合处理,PRF组智齿拔除术后于牙槽窝内填塞PRF,并进行常规缝合处理。比较两组疼痛程度、张口受限程度、相邻下颌第二磨牙远中牙周附着丧失(AL)、口腔健康及并发症。结果术后第1、3天,PRF组视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。PRF组张口受限程度均优于对照组,PRF组相邻第二磨牙远中牙周无AL占比高于对照组,AL≤2 mm占比低于对照组,差异有统计学意义(P<0.05);两组相邻第二磨牙远中牙周AL>2 mm占比比较差异无统计学意义。术后,PRF组口腔健康影响程度量表(OHIP-14)评分低于对照组,差异有统计学意义(P<0.05)。PRF组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论PRF用于下颌智齿拔除术中安全可行,可减轻患者肿胀、疼痛及张口受限程度,减少相邻第二磨牙远中牙周AL,促进牙周组织愈合,且术后并发症少,患者接受度高,值得临床推广应用。Objective To investigate the effect of platelet-rich fibrin(PRF)on wound prognosis in mandibular wisdom tooth extraction.Methods 50 patients with mandibular wisdom tooth extraction in Ganzhou Dermatology Hospital from October 2020 to October 2021 were selected as the research subjects,and they were divided into the PRF group and the control group by random number table method,with 25 cases in each group.The control group was treated with conventional suture after wisdom tooth extraction,and the PRF group was treated with PRF filling in the alveolar fossa after wisdom tooth extraction and conventional suture.Pain degree,mouth opening limitation,the distal periodontal attachment loss(AL)of the adjacent second molar was reduced,oral health and complications of adjacent mandibular second molars were compared between the two groups.Results 1st and 3rd d after operation,the visual analogue scale(VAS)scores in the PRF group were lower than those in the control group,and the differences were statistically significant(P<0.05).The degree of limitation of mouth opening in the PRF group was better than that in the control group.The proportion of no AL in the distal periodontal ligament of the adjacent second molar in the PRF group was higher than that in the control group,and the proportion of AL≤2 mm in the distal periodontal ligament in the PRF group was lower than that in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in the proportion of distal periodontal AL>2 mm between the two groups.After operation,The postoperative oral health impact profile(OHIP-14)score in the PRF group was lower than that in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the PRF group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion PRF is safe and feasible for mandibular wisdom tooth extraction,which can reduce swelling,pain and restricted mouth opening
