外周血细胞因子对非小细胞肺癌的预后及免疫治疗的疗效的预测作用

摘　　要：目的分析外周血中细胞因子在预测非小细胞肺癌(NSCLC)预后和免疫治疗疗效中的作用。方法回顾性分析2019年12月至2021年12月郑州大学第一附属医院收治的NSCLC患者共86例的临床资料,结合免疫治疗疗效分成有反应组(n=50)和无反应组(n=36),依据预后水平分成生存组(n=70)和死亡组(n=16),比较有反应组和无反应组、生存组和死亡组的基线资料、外周血细胞因子水平,有差异项目通过Logistic回归方程开展分析,观察外周血中细胞因子对NSCLC预后和免疫治疗疗效中的预测价值。结果有反应组和无反应组、生存组和死亡组的性别、年龄、吸烟史、饮酒史以及疾病分期等基线资料,与IL-2、IL-4、IL-6、IL-8、IL-10、IL-12、IL-13、TGF-β以及TNF-α水平相比,差异无统计学意义(P>0.05),但IL-5、IFN-γ水平相比,差异有统计学意义(P<0.05)。有差异项目通过Logistic回归方程开展分析,发现IL-5、IFN-γ均是NSCLC免疫治疗疗效和预后的预测因子,差异有统计学意义(P<0.05)。结论IL-5、IFN-γ能提供出NSCLC患者免疫治疗疗效以及预后信息,可作为免疫治疗疗效以及预后的重要预测指标。Objective To analyze the role of cytokines in peripheral blood in predicting the prognosis and immunotherapy efficacy of non-small cell lung cancer(NSCLC).Methods The clinical data of 86 patients with NSCLC admitted to the First Affiliated Hospital of Zhengzhou University from December 2019 to December 2021 were retrospectively analyzed.The patients were divided into response group(n=50)and non-response group(n=36)combined with immunotherapy efficacy,and divided into survival group(n=70)and death group(n=16)according to prognostic level.Baseline data and peripheral blood cytokine levels of the responding and non-responding groups,survival and death groups were compared,and items with differences were analyzed by Logistic regression equation to observe the predictive value of peripheral blood cytokines in NSCLC prognosis and immunotherapy efficacy.Results There was no significant difference in the baseline data of gender,age,smoking history,drinking history and disease stage between response group and non-response group,survival group and death group compared with the levels of IL-2,IL-4,IL-6,IL-8,IL-10,IL-12,IL-13,TGF-β and TNF-α(P>0.05).However,the levels of IL-5 and IFN-γ were significantly different(P<0.05).Logistic regression equation was used to analyze the items with differences,and it was found that IL-5 and IFN-γwere both predictors of immunotherapy efficacy and prognosis of NSCLC,and the difference was statistically significant(P<0.05).Conclusion IL-5 and IFN-γcan provide information about immunotherapy efficacy and prognosis of NSCLC patients,and can be used as important predictors of immunotherapy efficacy and prognosis.
