白内障超声乳化联合人工晶状体植入术在青光眼合并白内障患者中的应用

摘　　要：目的探讨白内障超声乳化(Phaco)联合人工晶状体(IOL)植入术用于治疗青光眼合并白内障患者效果。方法选取2020年8月至2021年7月光山县人民医院收治的94例青光眼合并白内障患者,按照随机数字表法分成研究组(47例)与对照组(47例),对照组采用小切口非超声乳化白内障摘除术(SICS)联合IOL植入术,研究组采用Phaco联合IOL植入术,比较两组眼部指标、眼压水平、氧化应激指标及并发症发生率。结果两组术后前房深度、房角开放度、最佳矫正视力明显高于术前,且研究组高于对照组;两组术后3 d、术后1个月、术后3个月眼压水平明显低于术前,且研究组低于对照组;两组术后超氧化物歧化酶(SOD)水平明显高于术前,丙二醛(MDA)、脂质过氧化物(LPO)水平明显低于术前,且研究组高于对照组SOD水平,MDA、LPO水平明显低于对照组;研究组并发症发生率(4.26%)低于对照组(19.15%),差异有统计学意义(P<0.05)。结论青光眼合并白内障患者给予Phaco联合IOL植入术治疗,能够改善眼部指标,降低眼压水平,改善患者氧化应激状况,减少并发症发生。Objective To investigate the application effect of phacoemulsification(Phaco)combined with intraocular lens(IOL)implantation in patients with glaucoma and cataract.Methods A total of 94 patients with glaucoma and cataract who were admitted in Guangshan County People’s Hospital from August 2020 to July 2021 were selected,and they were divided into the research group(47 cases)and the control group(47 cases)by the random number table method.The control group was treated with small incision non-phacoemulsification cataract extraction(SICS)combined with intraocular lens(IOL)implantation,and the research group were treated with cataract Phaco combined with IOL implantation.The ocular indicators,intraocular pressure levels,oxidative stress indicators and complication rates were compared of the two groups.Results After operation,the anterior chamber depth,chamber angle opening and best corrected visual acuity of the two groups were improved,and the research group was higher than the control group.The intraocular pressure level at 3 d,1 month,and 3 months after the operation was decreased,and the research group was lower than the control group.After operation,malonaldehyde(MDA)and lipid peroxide(LPO)levels were decreased,compared with the control group,the SOD levels in the research group were higher,and the MDA and LPO levels were lower.The incidence of complications in the research group(4.26%)was lower than the control group(19.15%),the difference is statistically significant(P<0.05).Conclusion Phacoemulsification combined with intraocular lens implantation in patients with glaucoma and cataract can improve ocular indexes,reduce intraocular pressure levels,improve oxidative stress indexes,and reduce the complications.
