摘 要:目的研究丝切蛋白(Cofilin)在神经胶质瘤中的表达及预后的关系。方法分析162例神经胶质瘤患者的组织芯片,采用免疫组织化学方法检测组织标本中Cofilin的表达水平,采用χ^(2)检验和Fisher精确概率法分析Cofilin阳性表达与阴性表达组间差异。采用Cox比例风险回归模型分析神经胶质瘤患者预后的危险因素,采用Kaplan-Meier法绘制患者总生存曲线。结果162例神经胶质瘤患者组织标本中,Cofilin阳性表达109例,阴性表达为53例。Cofilin阳性表达与年龄(χ^(2)=17.81,P<0.05)、病理分级(χ^(2)=12.67,P<0.05)、肿瘤复发情况(χ^(2)=141.41,P<0.05)及程序性死亡受体-1配体(PD-L1)(χ^(2)=6.21,P<0.05)表达比较,差异有统计学意义。单因素Cox回归分析发现年龄[风险比(HR):2.660,95%可信区间(CI):1.571~4.502,P<0.01]、病理分级(HR:16.463,95%CI:7.404~36.606,P<0.01)、细胞核增殖抗原(Ki-67)(HR:1.927,95%CI:1.137~3.265,P<0.05)、Cofilin阳性表达(HR:3.515,95%CI:1.661~7.441,P<0.01)是神经胶质瘤患者预后不良的危险因素。多因素Cox回归分析发现年龄(HR:2.310,95%CI:1.348~3.958,P<0.01)、病理分级(HR:6.371,95%CI:4.182~9.706,P<0.01)是神经胶质瘤患者预后不良的独立危险因素,采用Kaplan-Meier生存分析结果显示,Cofilin的阳性表达与较低的生存率明显相关(P<0.05)。结论Cofilin在神经胶质瘤在组织中阳性表达可提示患者预后差。Objective To investigate the relationship between the expression and prognosis of Cofilin in glioma.Methods Tissue microarrays of 162 patients with glioma were retrospectively analyzed,and the expression levels of Cofilin in tissue specimens were detected by immunohistochemistry,and the chi-square test and Fisher’s exact probability method were applied to analyze the differences between groups.The Cox proportional risk regression model was used to analyze the risk factors for prognosis of glioma patients,and the Kaplan-Meier method was applied to plot the patient overall survival curves.Results Among 162 cases of glioma tumor tissues,109 cases had Cofilin positive expression and 53 cases had negative expression.Cofilin positive expression and age(χ^(2)=17.81,P<0.05),pathological grading(χ^(2)=12.67,P<0.05),tumor recurrence(χ^(2)=141.41,P<0.05)and programmed death receptor-1 ligand(PD-L1)(χ^(2)=6.21,P<0.05).Univariate Cox regression analysis showed that age[hazard ratio(HR):2.660,95%confidence interval(CI):1.571-4.502,P<0.01],pathological grade(HR:16.463,95%CI:7.404-36.606,P<0.01),nuclear proliferation antigen(Ki-67)(HR:1.927,95%CI:1.137-3.265,P<0.05),and positive expression of Cofilin(HR:3.515,95%CI:1.661-7.441,P<0.01)were risk factors for poor prognosis of patients with glioma.Multivariate Cox regression analysis showed that age(HR:2.310,95%CI:1.348-3.958,P<0.01)and pathological grade(HR:6.371,95%CI:4.182-9.706,P<0.01)were independent risk factors for poor prognosis of patients with glioma.Kaplan-Meier survival analysis showed that the positive expression of Cofilin was associated with lower survival rate(P<0.05).Conclusion The positive expression of Cofilin in glioma tissue may indicate poor prognosis of patients.
