1例胰腺术后胃瘫并发Wernicke脑病患者的营养序贯治疗

摘　　要：韦尼克脑病(Wernicke encephalopathy,WE)是由于维生素B_(1)缺乏所引起的中枢神经系统急性或亚急性综合征,常见于慢性酒精中毒,少数可见于急性胰腺炎、神经性厌食、腹部手术、血液透析、妊娠剧吐等疾病,典型表现为“眼球麻痹、共济失调和意识障碍”三联征。早期诊断并及时以大剂量维生素B_(1)治疗能有效阻止疾病进展,使症状好转甚至痊愈。但多数患者无典型的临床表现,部分患者易被临床医生忽视或误诊,延误治疗,导致严重致残,甚至死亡。天津市第三中心医院2021年2月3日收治了一名胰腺术后胃瘫并发韦尼克脑病患者,经治疗后好转出院,现对治疗过程进行总结,以供临床参考。Wernicke encephalopathy(WE)is an acute or subacute syndrome of the central nervous system caused by vitamin B_(1) deficiency,which is commonly found in chronic alcoholism,and is rarely seen in acute pancreatitis,anorexia nervosa,abdominal surgery,hemodialysis induction,hyperemesis gravidarum and other diseases.The typical manifestation is"eyeball paralysis,ataxia and consciousness disorder"triad.Early diagnosis and timely treatment with large dose of vitamin B_(1) can effectively prevent the progression of the disease,improve the symptoms and even cure.However,most patients have no typical clinical manifestations,and some patients are easy to be ignored or misdiagnosed by clinicians,resulting in delayed treatment,leading to serious disability or even death.On February 3,2021,a patient with postoperative gastroparesis complicated with Wernicke encephalopathy was admitted to our hospital.After treatment,he was discharged from hospital with improvement.Here is a summary of the treatment process for clinical reference.
