S100可吸收止血绫在牙槽外科中的应用体会

摘　　要：目的分析复杂牙拔除手术中应用S100可吸收止血绫的临床效果。方法纳入2021年1月-2022年12月兰州大学第二医院口腔颌面外科接诊的100例复杂难拔牙患者的临床资料。按照术中是否应用S100进行分组,对照组41例(78颗)术中采用常规的止血措施,观察组59例(157颗)术中采用常规的止血措施联合S100。比较两组愈合情况(愈合良好率)、干槽症发生率、并发症发生率(过敏及拔除牙相关的不良事件)。结果术后7d随诊:两组愈合情况比较,差异有统计学意义,Z=6.230,P<0.05。观察组愈合良好率高于对照组,χ^(2)=4.970,P<0.05。观察组和对照组分别有1例/2颗患牙(占1.27%,2/157),6例/11颗患牙(占14.10%,11/78)在拔牙创口出现干槽症,两组干槽症发生率比较,差异有统计学意义,χ^(2)=5.120,P=0.024。结论在复杂牙拔除手术中应用S100,可以在止血的同时消灭死腔,进而防止积血积液,降低术后感染的风险。Objective To analyze the clinical effect of using S100 absorbable hemostatic material(S100)in complex tooth extraction surgery.Methods The clinical data of 100 patients with complicated and difficult tooth extraction who were treated in oral and maxillofacial surgery of the Second Hospital of Lanzhou University from January 2021 to December 2022 were included.Grouped according to whether S100 was used during surgery,41 cases(78 cases)in the control group received routine hemostatic measures during surgery,while 59 cases(157 cases)in the observation group received routine hemostatic measures combined with S100 during surgery.Compare the healing status(good healing rate),incidence of dry socket disease,and incidence of complications(allergies,adverse events related to tooth extraction)between the two groups.Results 7-day follow-up after surgery:the healing status of the two groups was compared and the difference was statistically significant,Z=6.230,P<0.05.The observation group had a higher rate of good healing than the control group,χ^(2)=4.970,P<0.05.One case/two affected teeth(1.27%,2/157)in the observation group developed dry socket syndrome at the extraction wound,while the control group had six cases/eleven affected teeth(14.10%,11/78).The difference in the incidence of dry socket syndrome between the two groups was statistically significant,χ^(2)=5.120,P=0.024.Conclusions The application of S100 in complex tooth extraction surgery can eliminate the dead space while stopping bleeding,thereby preventing blood and fluid accumulation,and reducing the risk of postoperative infection.
