基于无监督聚类分析法的ICU护士认知工作复杂性测评工具的分级研究

摘　　要：目的探索ICU护士认知工作复杂性测评工具(CAMEOⅡ)的分级标准。方法纳入2021年6-8月上海市第一人民医院208例ICU患者,运用无监督聚类分析法,基于CAMEOⅡ得分及患者疾病累及脏器数构建CAMEOⅡ分级标准。结果CAMEOⅡ得分与患者疾病累及脏器数呈显著正相关(P<0.05),与性别、年龄、BMI未见相关。根据聚类结果建立CAMEOⅡ分级标准:Ⅰ级为护理认知工作复杂程度较低,CAMEOⅡ分值<53分;Ⅱ级为护理认知工作复杂程度一般,CAMEOⅡ分值53~80分;Ⅲ级为护理认知工作复杂程度较高,CAMEOⅡ分值81~110分;Ⅳ级为护理认知工作复杂程度极高,CAMEOⅡ分值>110分。结论CAMEOⅡ分级标准的建立具有科学性,能为ICU护理人力资源配置测评系统的规范建立奠定研究基础。Objective To explore the grading standard of the complexity assessment and monitoring to ensure optimal outcomes(CAMEOⅡ)for ICU nurses,so as to provide basis for scientific allocation of ICU nursing human resource.Methods A total of 208 ICU patients in a tertiary hospital in Shanghai from June to August 2021 were included,and the classification standard of CAMEOⅡwhich was constructed based on the score of CAMEOⅡand the number of organs involved in the disease by unsupervised cluster analysis.Results CAMEOⅡwas significantly positively correlated with the number of organs involved in the disease(P<0.05),but not with gender,age,and BMI.The classification standard of CAMEOⅡwas established according to the clustering results:GradeⅠwas that the complexity of nursing cognitive work was low,and the score of CAMEOⅡwas<53 points;GradeⅡwas that the complexity of nursing cognitive work was general,and the score of CAMEOⅡwas 53-80 points;GradeⅢwas that the complexity of nursing cognitive work was high,and the score of CAMEOⅡwas 81-110 points;GradeⅣwas that the complexity of nursing cognitive work was very high,and the score of CAMEOⅡwas>110 points.Conclusion The establishment of CAMEOⅡclassification standard is scientific,which can lay a research foundation for the standardized establishment of ICU nursing human resource allocation evaluation system.
