摘　　要：目的:探讨不同检测方式在宫颈病变筛查中的应用价值。方法:选取2021年4月—2022年2月于开平市中心医院行宫颈脱落细胞血常规检查患者700例作为研究对象,采取随机分组法分为液基薄层细胞学(TCT)组、人乳头状瘤病毒(HPV)组、DNA倍体细胞检测(ICM-DNA)组、TCT+HPV组、TCT+ICM-DNA组、HPV+ICM-DNA组、TCT+HPV+ICM-DNA组,各100例。TCT组、HPV组、ICM-DNA组、TCT+HPV组、TCT+ICM-DNA组、HPV+ICM-DNA组、TCT+HPV+ICM-DNA组分别应用TCT、HPV、ICM-DNA、TCT+HPV、TCT+ICM-DNA、HPV+ICM-DNA、TCT+HPV+ICM-DNA检查。对检查阳性者行阴道镜检查,以宫颈组织病理活检为诊断标准,对各组阳性检出率、敏感度、特异度、阳性预测值、阴性预测值、诊断符合率进行比较分析。结果:TCT+HPV组、TCT+ICM-DNA组、HPV+ICM-DNA组、TCT+HPV+ICM-DNA组联合检查的阳性检出符合率均高于TCT组、HPV组、ICM-DNA组单一检查;TCT+HPV+ICM-DNA组阳性检出符合率显著高于其他组,且与各单一检查组相比,差异有统计学意义(P<0.05)。TCT+HPV组、TCT+ICM-DNA组、HPV+ICM-DNA组、TCT+HPV+ICM-DNA组的灵敏度、特异度、阳性预测值、阴性预测值、诊断符合率均高于TCT组、HPV组、ICM-DNA组;TCT+HPV+ICM-DNA组特异度、阳性预测值、阴性预测值、诊断符合率优于各单一检查组,差异有统计学意义(P<0.01)。结论:与单一检测法比较,TCT、HPV、ICM-DNA联合筛查宫颈病变具有良好的敏感度、特异度、可接受性,可帮助临床提高早期诊断检出率。Objective:To explore the application value of different detection methods in cervical lesions screening.Methods:From April 2021 to February 2022,700 patients with cervical exfoliated cells in Kaiping Central Hospital were randomly divided into TCT group,HPV group,ICM-DNA group,TCT+HPV group,TCT+ICM-DNA group,HPV+ICM-DNA group and TCT+HPV+ICM-DNA group,with 100 patients in each group.TCT group,HPV group,ICM-DNA group,TCT+HPV group,TCT+ICM-DNA group,HPV+ICM-DNA group,TCT+HPV+ICM-DNA group,TCT+HPV+ICM-DNA group were examined with TCT,HPV,ICMDNA,TCT+HPV,TCT+ICM-DNA,HPV+ICM-DNA,TCT+HPV+ICM-DNA respectively.The positive rate,sensitivity,specificity,positive predictive value,negative predictive value and diagnostic coincidence rate of each group were compared and analyzed.Results:The positive detection coincidence rate of combined examination of TCT+HPV group,TCT+ICM-DNA group,HPV+ICM-DNA group and TCT+HPV+ICM-DNA group was higher than that of single examination of TCT group,HPV group and ICM-DNA group;the coincidence rate of positive detection in TCT+HPV+ICM-DNA group was significantly higher than that in other groups,and the difference was statistically significant(P<0.05);the sensitivity,specificity,positive predictive value,negative predictive value and diagnostic coincidence rate of TCT+HPV group,TCT+ICM-DNA group,HPV+ICM-DNA group and TCT+HPV+ICM-DNA group were higher than those of TCT group,HPV group and ICM-DNA group.The specificity,positive predictive value,negative predictive value and diagnostic coincidence rate of TCT+HPV+ICM-DNA group were superior to those of single examination group,and the differences were statistically significant(P<0.01).Conclusion:Compared with single detection method,TCT,HPV and ICM-DNA combined screening of cervical lesions has good sensitivity,specificity and acceptability,which can help clinical improve the detection rate of early diagnosis.
