直接盖髓术及活髓切断术治疗龋源性有症状不可复性牙髓炎效果分析

摘　　要：目的对比探讨直接盖髓术、活髓切断术对龋源性有症状不可复性牙髓炎的疗效。方法选择医院2019年1月至2021年1月诊治的龋源性有症状不可复性牙髓炎患者80例,使用简单随机数分组法将所有患者分成对照组和观察组,例数各40例。对照组患者接受直接盖髓术治疗,研究组患者进行活髓切断术治疗。比较2组患者修复成功情况、疼痛和操作时间、满意度。结果研究组患者在术后3个月42%、6个月修复成功率72.50%高于对照组(P<0.05),术后12个月修复成功率与对照组比较差异无统计学意义(P>0.05)。术后2周,2组VAS评分低于术前(P<0.05)。术后2周,研究组患者VAS(1.06±0.21)分低于对照组(P<0.05)。研究组患者操作时间(25.61±2.14)min与对照组(24.69±2.06)min比较差异无统计学意义(P>0.05)。研究组患者总满意率87.50%高于对照组70%(P<0.05)。结论活髓切断术较直接盖髓术在龋源性有症状不可复性牙髓炎上能获得更好的修复效果,建议使用。Objective To discuss the clinical efficacy of direct pulp capping and vital pulpotomy in the treatment of cariogenic and symptomatic irreversible pulpitis.Methods Eighty patients with cariogenic and symptomatic irreversible pulpitis in our hospital from January 2019 to January 2021 were enrolled,and classified into two groups using random num⁃ber table methods,each with 40 cases.Control group received direct pulp capping,while study group received vital pulpoto⁃my.Then comparison was conducted on the success rate of repair,pain degree,operation time and satisfaction rate between groups.Results The repair success rates at postoperative 3 and 6 months were 42%and 72.50%in study group,and were remarkably higher than those of control group(P<0.05),while no statistical difference was found in the success rate at postoperative 12 months(P>0.05).A decrease in Visual Analogue Scale(VAS)score was observed in both groups at post⁃operative 2 weeks,and the score was(1.06±0.21)in study group,which was higher than control group(P<0.05).The op⁃eration time of study group yielded no statistical difference with that of control group[(25.61±2.14)vs(24.69±2.06),P>0.05].The overall satisfaction rate was 87.50%in study group,and was higher than 70%in control group(P<0.05).Conclusion Compared with direct pulp capping,application of vital pulpotomy in the treatment of cariogenic and symptomatic irreversible pulpitis has better repair effect,which is worthy of recommendation.
