摘 要:目的 研究夜磨牙、口呼吸、偏侧咀嚼等不良习惯与错颌畸形之间的关系,为预防牙科疾病提供依据。方法 选取我院2020年1月-2022年1月期间于我院口腔科就诊并确诊为错颌畸形儿童患者60例作为观察组,另选同期牙科检查正常者49例作为对照组,对比两组人员有无夜磨牙、口呼吸、偏侧咀嚼等不良习惯,并使用logistic回归模型分析夜磨牙、口呼吸、偏侧咀嚼与错颌畸形之间的关系。结果 本次研究观察组患儿60例错颌畸形中安氏Ⅰ类31人,占比51.66%,其中牙列不齐18人,占比30.00%,牙弓前突8人,占比13.33%牙列间隙5人,占比8.33%;安氏Ⅱ类17人,占比28.33%,其中深覆盖12人,占比20.00%,深覆合5人,占比8.33%;安氏Ⅲ类12人,占比20.00%,其中牙性反合7人,占比11.66%,上颌骨发育不足5人,占比8.33%。对错颌畸形的影响作单因素分析,夜磨牙、口呼吸、偏侧咀嚼均为错颌畸形的独立危险因素(P<0.05)。logistic回归分析结果得出夜磨牙OR=2.776(95%CI:1.692~5.475),口呼吸OR=1.841(95%CI:1.436~3.662),偏侧咀嚼OR=3.195(95%CI:1.879~7.160),且偏侧咀嚼在回归中OR值最高,是对错颌畸形影响最大的因素之一。结论 夜磨牙、口呼吸、偏侧咀嚼均为错颌畸形的独立危险因素,其中偏侧咀嚼影响最大,应该及时敦促儿童改正该类不良用牙习惯,以防止错颌畸形的形成。Objective To study the relationship between malocclusion and bad habits such as night bruxism,mouth breathing,and hemi⁃mastication,so as to provide evidence for the prevention of dental diseases.Methods Sixty children with malocclusion who visited the Department of Stomatology of our hospital from January 2021 to January 2022 were select⁃ed as the observation group,and another 49 patients with normal dental examination in the same period were selected as the control group.The presence or absence of bad habits such as night bruxism,mouth breathing,and hemi⁃mastication was com⁃pared between the two groups,and the relationship between night bruxism,mouth breathing,hemi⁃mastication,and malocclu⁃sion was analyzed using a logistic regression model.Results In this study,31 of 60 cases(51.66%)of malocclusion in the observation group were Angle Class Ⅰ,including 18 cases(30.00%)of irregular dentition,5 cases(8.33%)of arch protru⁃sion,8 cases(13.33%)of anterior retrognathia;17 cases(28.33%)of Angle Class Ⅱ,including 12 cases(20.00%)of deep overjet,5 cases(8.33%)of upper anterior inclination;12 cases(20.00%)of Angle Class Ⅲ,including 7 cases(11.66%)of jaw protrusion and 5 cases(8.33%)of anti⁃jaw.Univariate analysis of the effects of malocclusion showed that night bruxism,mouth breathing,and unilateral mastication were all independent risk factors for malocclusion(P<0.05).The results of logistic regression analysis showed that OR=2.776(95%CI:1.692~5.475)was for night bruxism,OR=1.841(95%CI:1.436~3.662)for mouth breathing,and OR=3.195(95%CI:1.879~7.160)for hemi⁃mastication,and hemi⁃mastica⁃tion had the highest OR in the regression,which was one of the factors with the greatest effect on malocclusion.Conclusion Night bruxism,mouth breathing and hemi⁃mastication are all independent risk factors of malocclusion,and hemi⁃mastica⁃tion has the greatest effect.Children should be urged to correct these bad tooth habits in time to prevent the formation of malocclusion.
