基于OCTA检测中高度近视青年黄斑区血流的研究

摘  要:目的 采用光学相干断层扫描血管造影(OCTA)检测中高度近视青年患者黄斑区血流密度的变化,探讨眼球各项参数与血流变化的相关性。方法 选取18~40岁中高度近视患者及正常人群60例(60眼)作为研究对象,根据等效球镜度数(SE)将纳入的眼数分为中高度近视组(-9.00 D≤等效球镜度数<-3.00 D)和正常对照组(等效球镜度数≥0 D),各30眼(30例)。所有研究对象均进行眼轴及OCTA检查,收集浅层毛细血管丛黄斑中心凹无血管区(SCP FAZ)面积、深层毛细血管丛黄斑中心凹无血管区(DCP FAZ)面积、浅层毛细血管丛黄斑中心凹微血管密度(SCP MVD)、深层毛细血管丛黄斑中心凹微血管密度(DCP MVD)、脉络膜毛细血管黄斑中心凹微血管密度(CC MVD)以及黄斑中心凹的视网膜厚度和脉络膜厚度。比较两组研究对象的一般资料(性别、年龄)、眼球参数(眼压、等效球镜度及眼轴、脉络膜厚度、视网膜厚度)、黄斑区OCTA结果 ,分析等效球镜度、眼轴与血流密度的相关性。结果 两组性别、年龄、眼压及视网膜厚度比较,差异无统计学意义(P>0.05);中高度近视组等效球镜度(-6.03±1.88)D低于正常对照组的(1.08±0.73)D,眼轴(26.28±1.38)mm长于正常对照组的(22.91±0.64)mm,脉络膜厚度(182.63±72.27)μm小于正常对照组的(313.47±45.13)μm,差异有统计学意义(P<0.05)。中高度近视组SCP MVD(9.73±2.50)、DCP MVD(8.90±1.70)、CC MVD(42.99±6.91)均小于正常对照组(13.34±1.62)、(10.77±2.72)、(48.29±8.26), SCP FAZ面积(0.57±0.47)mm^(2)及DCP FAZ面积(0.89±0.22)mm^(2)均大于正常对照组的(0.36±0.10)、(0.68±0.20)mm^(2),差异有统计学意义(P<0.05)。Pearson相关分析显示, SCP MVD、DCP MVD、CC MVD与等效球镜度和脉络膜厚度呈正相关性(r=0.769、0.439、0.366和0.928、0.525、0.272, P<0.05),与眼轴长度呈负相关(r=-0.791、-0.454、-0.336, P<0.05)。SCP FAZ面积、DCP FAZ面积与等效球镜度和脉络膜厚度�Objective To investigate the changes of the macular blood flow in youth patients with moderate-to-high myopia by optical coherence tomography angiography(OCTA),and the correlation between the ocular parameters and blood flow changes.Methods 60 cases(60 eyes)of middle-to-high myopia patients aged 18-40 years and normal population were selected as the study subjects.According to spherical equivalent(SE),the included eyes were divided into medium-to-high myopia group(-9.00 D≤spherical equivalent<-3.00 D)and normal control group(≥0 D spherical equivalent),each with 30 eyes(30 cases).All subjects underwent ocular axis and OCTA examination.The area of foveal avascular zone in superficial capillary plexus(SCP FAZ),area of foveal avascular zone in deep capillary plexus(DCP FAZ),macular vascular density in superficial capillary plexus(SCP MVD),macular vascular density in deep capillary plexus(DCP MVD),macular vascular density in the choriocapillaris(CC MVD),foveal retinal thickness and foveal choroidal thickness were measured.The general data(gender,age),ocular parameters(intraocular pressure,spherical equivalent and axial length,choroidal thickness,retinal thickness)and OCTA results of macular area of the two groups were compared,and the correlation of spherical equivalent and ocular axis with blood flow density was analyzed.Results There was no statistically significant difference in gender,age,intraocular pressure and retinal thickness between the two groups(P>0.05).The spherical equivalent of(-6.03±1.88)D in the medium-to-high myopia group was lower than that of(1.08±0.73)D in the normal control group;the axial length of(26.28±1.38)mm in the medium-to-high myopia group was longer than that of(22.91±0.64)mm in the normal control group;the choroidal thickness of(182.63±72.27)μm in the medium-to-high myopia group was lower than that of(313.47±45.13)μm in the normal control group;the differences were statistically significant(P<0.05).The medium-to-high myopia group had SCP MVD of(9.73±2.50),DCP MVD of(8.90±1
