阿替普酶静脉溶栓治疗轻型缺血性脑卒中的临床研究

摘　　要：目的 分析临床治疗轻型缺血性脑卒中(MIS)患者期间,予以阿替普酶静脉溶栓治疗的应用价值探究。方法 选取2022年3月至2023年3月福建省立医院临床诊治的轻型缺血性脑卒中患者90例,根据随机数表法分为3组,溶栓1组(轻型非致残性卒中患者)、溶栓2组(轻型致残性卒中患者)均使用阿替普脉溶栓治疗,对照组使用常规治疗,每组各30例,分析治疗效果、不良反应、国际通用国立卫生院卒中量表(NIHSS)评分、改良Rankin量表(mRS)评分、Barthel评分、血清指标。结果 溶栓1组患者总有效率为(96.67%)高于溶栓2组(80.00%)、对照组(53.33%),溶栓1组不良反应总发生率(0)低于溶栓2组(13.33%)、对照组(36.67%),溶栓1组患者NIHSS评分、mRS评分和血清指标低于溶栓2组、对照组患者,Barthel评分高于溶栓2组、对照组,差异均有统计学意义(P<0.05)。结论 治疗患者轻型缺血性脑卒中症状期间,予以阿替普酶静脉溶栓治疗,能够减少不良反应,缓解患者神经功能受损情况,提高患者独立能力,值得临床广泛推荐使用。Objective To analyze the application value of alteplase intravenous thrombolysis therapy during the clinical treatment of patients with mild ischemic stroke(MIS).Methods A total of 90 patients with mild ischemic stroke in the clinical diagnosis and treatment of Fujian Provincial Hospital were included in the study.The study period was from March 2022 to March 2023.The selected data were randomly divided into three groups using a number table method.Thrombolysis group 1(mild non-disabling stroke patients)and thrombolysis group 2(mild disabling stroke patients)were all treated with alteplase intravenous thrombolysis,while the control group was treated with conventional therapy,with 30 patients in each group.The treatment effect,adverse reactions,national institute of health stroke scale(NIHSS)score,modified Rankin scale(mRS)score,Barthel score,serum indicators,etc.were analyzed.Results The treatment effect of thrombolysis group 1(96.67%)was higher than that of thrombolysis group 2(80.00%)and control group(53.33%).The adverse reactions of thrombolysis group 1(0.00%)were lower than those of thrombolysis group 2(13.33%)and control group(36.67%).The NIHSS score,the mRs score and seru indicatots of patients in thrombolysis group 1 was lower than that of patients in thrombolysis group 2 and control group,with satistically significant differences(P<0.05).The Barthel scores of patients in thrombolysis group 1 were higher than those in thrombolysis group 2 and control group,with statistically significant difference(P<0.05).Conclusion During the treatment of mild ischemic stroke symptoms in patients,intravenous thrombolysis with alteplase can reduce adverse reactions,alleviate neurological impairment,and improve patient independence.It is widely recommended for clinical use.
