摘　　要：目的初步探讨原发性膀胱小细胞癌(SCCB)诊治方案,提高对膀胱细胞癌的临床认知。方法对1例SCCB患者的临床、病理资料及诊疗经过进行回顾性分析。患者中年男性,于2021年10月接受经尿道膀胱肿瘤电切术(TURBT),病理检查确诊为SCCB,术后予以吉西他滨膀胱灌注+辅助放疗等局部治疗后发生肺部及肋骨转移,改用全身化疗联合替雷利珠单抗免疫治疗,完成化疗周期后,予以免疫维持治疗。结果患者肺部转移灶消退、肋骨及组织旁肿块缩小,根据实体肿瘤疗效评估标准1.1(RECIST1.1)评估达到部分缓解(PR)。结论SCCB是恶性程度极高的泌尿系肿瘤,单纯局部治疗难以控制,对于复发或转移性SCCB患者可考虑免疫联合化疗治疗。Objective To preliminarily investigate the diagnosis and treatment of primary small cell carcinoma of bladder(SCCB),and enhance the clinical cognition of bladder cell carcinoma.Methods The clinical and pathological data and diagnosis and treatment of a patient with SCCB were analyzed retrospectively.A middle-aged male patient underwent transurethral resection of bladder tumor(TURBT)in October 2021,and was diagnosed as small cell carcinoma of bladder by pathological examination.After the operation,he was given local treatment such as intravesical instillation of gemcitabine and adjuvant radiotherapy,and his lungs and ribs metastasized.He switched to systemic chemotherapy combined with immunotherapy with tislelizumab,and after completing the chemotherapy cycle,he was given immune maintenance treatment.Results The patient’s lung metastasis subsided,and the mass around ribs and tissues shrank.According to the response evaluation criteria in solid tumors 1.1(RECIST1.1),partial remission(PR)has been achieved.Conclusion SCCB is a highly malignant urinary tumor,which is difficult to control by local treatment alone.For patients with relapse or metastatic SCCB,immunotherapy combined with chemotherapy can be considered.
