舌下腺囊肿袋成型术临床疗效的评价

摘　　要：目的:探究舌下腺囊肿(SGC)袋成型术的临床疗效。方法:收集我院2021年7月~2022年12月在我院口腔科就诊的口内型舌下腺囊肿患者80例临床资料。根据术式的不同将80例患者分为舌下腺囊肿袋组(n=42)和传统术式组(n=38)。舌下腺囊肿袋组行舌下腺囊肿袋成型术,传统术式组行舌下腺切除术。比较两组患者治疗效果、手术相关指标(手术时间、术中出血量、术后24 h疼痛程度)、疼痛因子[血清P物质(SP)、五羟色胺(5-HT)、去甲肾上腺素(NE)]以及并发症(颌下腺导管损伤、血肿与出血、舌体麻木、术后感染)的发生率。结果:传统术式组治愈人数高于舌下腺囊肿袋组,但舌下腺囊肿袋组临床治愈率与传统术式组相近(P>0.05)。舌下腺囊肿袋组患者手术时间、术中出血量、术后24 h疼痛程度均低于传统术式组(P<0.05)。治疗后两组患者术后1d的疼痛因子水平较术前均有所上升,但舌下腺囊肿袋组疼痛因子水平低于传统术式组(P<0.05)。舌下腺囊肿袋组患者发生颌下腺导管损伤、血肿与出血、舌体麻木、术后感染等总并发症发生率低于传统术式组(P<0.05)结论:舌下腺囊肿袋成型术不仅可以缩短患者的手术时间,缓解术后疼痛,促进术后恢复,还可以降低并发症发生的可能性,对舌下腺囊肿的治疗效果显著,具有可行性。Objective:To investigate the clinical effect of sublingual gland cyst(SGC)bag molding.Methods:The clinical data of 80 patients with intraoral sublingual gland cyst in our hospital from July 2021 to December 2022 were collected.80 patients were divided into sublingual gland cyst pocket group(n=42)and traditional group(n=38)according to different operation methods.Sublingual cyst bag group underwent sublingual cyst bag molding,traditional operation group underwent sublingual gland resection.The incidence of treatment effect,operative related indexes(operation time,intraoperative blood loss,postoperative pain degree 24 h),pain factors(serum Substance P(SP),serotonin(5-HT),norepinephrine(NE))and complications(submaxillary gland duct injury,hematoma and hemorrhage,tongue numbness,postoperative infection)were compared between the two groups.Results:The number of patients cured in the traditional operation group was higher than that in the sublingual cyst bag group,but the clinical cure rate in the sublingual cyst bag group was similar to that in the traditional operation group(P>0.05).The operative time,intraoperative blood loss and 24 h postoperative pain in sublingual gland cyst bag group were lower than those in traditional group(P<0.05).After treatment,the level of pain factor in 1 day after surgery in both groups was higher than that before surgery,but the level of pain factor in sublingual cyst bag group was lower than that in traditional operation group(P<0.05).The incidence of submandibular duct injury,hematoma and hemorrhage,tongue numbness and postoperative infection in sublingual gland cyst bag group was lower than that in traditional operation group(P<0.05).Conclusion:Bag molding of sublingual gland cyst can not only shorten the operation time of patients,relieve postoperative pain,promote postoperative recovery,but also reduce the possibility of complications.The treatment effect of sublingual gland cyst is significant and feasible.
