摘 要:目的:观察甘草酸单铵半胱氨酸联合谷胱甘肽(Glutathione,GSH)治疗抗结核药物性肝损伤(Anti-tuberculosis drug-induced liver injury,ATB-DILI)的疗效。方法:选取2020年3月至2022年3月间本院接收的80例由抗结核药引起药性肝损伤患者,根据信封随机法分为对照组和联合组,每组40例。对照组给予GSH治疗,联合组在前者基础上给予甘草酸单铵半胱氨酸治疗。比较两组患者的临床疗效、肝功能指标和血清炎性指标,统计不良反应发生率。结果:治疗后联合组总有效率明显高于对照组(P<0.05)。治疗后两组患者血清谷丙转氨酶(Glutamic pyruvic transaminase,ALT)、谷草转氨酶(Glutamic oxaloacetic transaminase,AST)、碱性磷酸酶(Alkaline phosphatase,ALP)、总胆红素(Total bilirubin,TBIL)、谷氨酰转移酶(Glutamyl transpeptidase,GGT)指标均下降,且联合组水平远低于对照组(P<0.05)。治疗后,两组患者白细胞介素-6(Interleukin-6,IL-6)和肿瘤坏死因子-α(Tumor necrosis factor,TNF-α)水平下降,且联合组水平低于对照组(P<0.05)。治疗期间联合组与对照组的不良反应总发生率比较无明显差异(P>0.05)。结论:甘草酸单铵半胱氨酸联合GSH治疗ATB-DILI疗效确切,有效改善肝功能指标和血清指标,并且没有不良反应的风险。Objective:To observe the therapeutic effects of monoammonium glycyrrhizinate and cysteine combined with glutathione(GSH)on ATB-DILI.Methods:A total of 80 patients with ATB-DILI admitted to the hospital from March 2020 to March 2022 were selected and randomly divided into a control group(40 patients treated with GSH)and a combination group(40 patients treated with monoammonium glycyrrhizinate and cysteine combined with GSH).Comparison between groups was made in terms of clinical effects and liver function indicators.The incidence of adverse reactions was calculated.Results:The total response rate of treatment in the combination group was significantly higher as compared with the control group(P<0.05).After treatment,serum ALT,AST,ALP,TBIL and GGT levels in both groups decreased,and the levels combination group were significantly lower as compared with the control group(P<0.05).After treatment,both groups of patients had interleukin-6(IL-6)and tumor necrosis factor-αTumor Necrosis factor,TNF-α).The level decreased,and the combined group had lower levels than the control group(P<0.05).The total incidence rates of adverse reactions in the two groups without statistically significant difference between the groups(P>0.05).Conclusion:Monoammonium glycyrrhizinate and cysteine combined with GSH is effective in treating ATB-DILI,which can effectively improve liver function indicators,without increasing adverse reactions.
