两种不同移植术联合胬肉切除治疗翼状胬肉的美学效果及对角膜上皮修复的影响

摘　　要：目的:探究角膜缘干细胞移植术与羊膜移植术联合胬肉切除对翼状胬肉患者角膜上皮修复的影响及其美学效果。方法:选取2019年3月-2021年3月于笔者医院就诊的82例翼状胬肉患者的临床资料,根据手术方式分为羊膜组(n=40)和角膜组(n=42)。统计两组患者围术期指标,比较两组患者术前及术后3个月眼表泪液指标、视力指标、角膜上皮功能,对比两组患者术后3个月后美学效果,并记录两组患者术后3个月内不良反应发生情况。结果:角膜组患者手术时长长于羊膜组(P<0.05);角膜组患者眼部刺激症状消失时间、角膜上皮修复时间短于羊膜组(P<0.05);术后3个月后,两组患者泪膜破裂时间(Break-up time,BUT)、基础泪液分泌实验(Schirmer I test,SIT)结果高于术前,且角膜组高于羊膜组(P<0.05);术后3个月后,两组患者裸眼视力(Uncorrectedvisualacuity,UCVA)、角膜散光度及角膜上皮荧光素染色评分(Fluorescent,FL)均低于术前,且角膜组低于羊膜组(P<0.05);术后3个月,角膜组美学优良率高于羊膜组(P<0.05);术后3个月内,两组患者不良事件发生率比较差异无统计学意义(P>0.05)。结论:角膜缘干细胞移植术与羊膜移植术在翼状胬肉患者的临床治疗中均可取得较好的治疗及美学效果,且可有效改善患者眼表泪液系统及角膜上皮功能,具有较好的安全性,但角膜缘干细胞移植术对患者各项指标改善效果更佳。Objective To explore the influence and aesthetic effect of limbal stem cell transplantation or amniotic membrane transplantation combined with pterygium excision on the corneal epithelial repair in patients with pterygium.Methods Clinical data of 82 patients with pterygium who were treated in the author's hospital from March 2019 to March 2021 were selected and the patients were divided into amniotic membrane group(n=40)and cornea group(n=42)according to the surgical methods.Perioperative indicators were statistically analyzed in the two groups.Ocular surface tear indicators,visual acuity indicators and corneal epithelial function before surgery and at 3 months after surgery and aesthetic effect at 3 months after surgery were compared between the two groups of patients.The adverse reactions were recorded in both groups within 3 months after surgery.Results The surgical duration in cornea group was longer than that in amniotic membrane group(P<0.05).The disappearance time of ocular irritation symptom and repair time of corneal epithelium in cornea group were shorter compared with those in amniotic membrane group(P<0.05).At 3 months after surgery,the break-up time(BUT)and Schirmer I test(SIT)in both groups were higher than those before surgery,and the indicators in cornea group were higher than those in amniotic membrane group(P<0.05).The uncorrected visual acuity(UCVA),corneal astigmatism and corneal epithelial fluorescein staining score(FL)in both groups at 3 months after surgery were lower than those before surgery,and the above indicators were lower in cornea group than those in amniotic membrane group(P<0.05).At 3 months after surgery,the excellent and good rate of aesthetics in cornea group was higher compared to amniotic membrane group(P<0.05).There were no statistical differences in the incidence rates of adverse events between the two groups within 3 months after surgery(P>0.05).Conclusion Limbal stem cell transplantation and amniotic membrane transplantation both can achieve good therapeutic and aestheti
