正畸配合牙槽骨再生术治疗成人中重度牙周炎错牙合畸形临床疗效分析

摘　　要：目的:探究正畸配合牙槽骨再生治疗成人中重度牙周炎错牙合畸形的临床疗效。方法:选取笔者医院2017年1月-2021年1月收治的102例中重度牙周炎错牙合畸形成人患者,采取正畸配合牙槽骨再生治疗。比较患者治疗前后牙周情况、牙齿松动度、咬合关系及美观性。结果:相较于治疗前,患者治疗后3个月及治疗后6个月的牙周探诊深度、附着丧失、龈沟出血指数值均逐渐减小,牙齿松动Ⅱ度及Ⅲ度例数、咬合时间、左右侧咬合力平衡度均逐渐减少(均P<0.05),牙齿松动Ⅰ度例数及咬合力占最大咬合力百分比均逐渐增加(P<0.05);患者治疗后6个月、治疗后1年,IOTN-AC美观评分、上下唇角及软组织面角均较治疗前逐渐降低(P<0.05),鼻唇角、颏唇沟角均逐渐增大(P<0.05)。结论:正畸配合牙槽骨再生治疗中重度牙周炎错牙合畸形患者效果显著,可有效改善患者牙槽骨吸收及牙齿松动情况,促进患者咬合功能恢复,提升面部美观度。Objective To explore the clinical efficacy and aesthetic effect of alveolar bone regeneration orthodontic treatment on adults with moderate-to-severe periodontitis with malocclusion.Methods 102 adults with moderate-to-severe periodontitis complicated with malocclusion who were treated in author’s hospital were selected between January 2017 and January 2021,and they received alveolar bone regeneration combined with orthodontic treatment.The periodontal status,tooth mobility,occlusal relation and aesthetics of patients were compared before and after treatment.Results Compared with before treatment,the periodontal probing depth,attachment loss,sulcus bleeding index,the number of cases with degreeⅡand degreeⅢtooth mobility,the occlusal time and bite force distribution balance of patients at 3 months and 6 months after treatment were gradually decreased(P<0.05).The number of cases with degree I tooth mobility and the maximum area bite force/max movie force were increased gradually(P<0.05).Compared with that before treatment,the patients'IOTN-AC aesthetic score,upper and lower lip angles and soft tissue facial angles decreased gradually 6 months after treatment and 1 year after treatment(P<0.05),and nasolabial angle and geniolabial groove angle increased gradually(P<0.05).Conclusion Alveolar bone regeneration orthodontic treatment has a signifi cant efficacy on patients with moderate-to-severe periodontitis and malocclusion.It can effectively improve the alveolar bone resorption and tooth mobility,promote the recovery of occlusal function,and enhance the facial aesthetic degree,with high clinical value.
