基于扎根理论的心理护理对面部烧伤患者自我残障接受度及创伤后成长的影响

摘　　要：目的:探讨基于扎根理论的心理护理对面部烧伤患者自我残障接受度、创伤后成长的影响。方法:选取82例面部烧伤患者,采用随机数字表法随机分为对照组和观察组,每组41例。对照组采取常规护理干预;观察组在对照组基础上给予基于扎根理论的心理护理。比较两组干预前后的负性情绪[焦虑自评量表(Self-ratomganxietyseale,SAS)和抑郁自评量表(Self rating depression scale,SDS)]、体像障碍程度(体像障碍自评量表)、创伤后应激障碍[创伤后应激障碍自评量表(PTSD checklist-civilian version,PCL-C)]、自我残障接受度[伤残接受度量表(Acceptance of disability scale,AODS)]以及创伤后自我成长[中文版创伤后成长评定量表(Chinese post-traumatic growth inventory,C-PTGI)]变化。结果:干预后,观察组SAS、SDS、体像障碍自评量表及PCL-C评分均低于对照组(P<0.05),AODS、C-PTGI评分均高于对照组(P<0.05)。结论:基于扎根理论的心理护理能明显改善面部烧伤患者的负性情绪、体像障碍程度、创伤后应激障碍,提升患者的自我残障接受度,促进患者创伤后成长。Objective To explore the effects of grounded theory-based psychological nursing on self-disability acceptance and post-traumatic growth in patients with facial burns.Methods Totally 82 patients with facial burns were selected and randomly divided into control group and observation group with 41 cases in each group by the random number table method.The control group received routine nursing intervention while the observation group was given grounded theory-based psychological nursing on the basis of the intervention in the control group.The negative emotions[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],degree of body image disorder(body image disorder self-rating scale),post-traumatic stress disorder[PTSD checklist-civilian version(PCL-C)],self-disability acceptance[acceptance of disability scale(AODS)]and post-traumatic self-growth[Chinese post-traumatic growth inventory(C-PTGI)]were compared between the two groups before and after intervention.Results After the intervention,the scores of SAS,SDS,Self-rating Scale of Body Image Disorder and PCL-C in the observation group were lower than those in the control group(P<0.05),and the scores of AODS and C-PTGI were higher than those in the control group(P<0.05).Conclusion Grounded theory-based psychological nursing can significantly improve the negative emotions,body image disorder degree and post-traumatic stress disorder,enhance the acceptance of self-disability,and promote the post-traumatic growth of patients with facial burns.
