子宫颈高级别上皮内瘤变患者宫颈锥切术后行腹腔镜下全子宫切除术的间隔时间研究

摘　　要：目的探讨子宫颈高级别上皮内瘤变患者宫颈锥切术后行腹腔镜下全子宫切除术的间隔时间。方法选取本院宫颈锥切术后行腹腔镜下全子宫切除术的46例子宫颈高级别上皮内瘤变患者为研究对象,根据宫颈锥切术后行腹腔镜下全子宫切除术的间隔时间分为三组,间隔72 h为A组(16例),间隔4~6周为B组(15例),间隔6~8周为C组(15例),比较三组的手术指标及术后并发症发生情况。结果三组的手术时间、术中出血量、术后白细胞计数、住院时间比较差异有统计学意义(P<0.05),且B组明显优于A、C组(P<0.05);B组术后并发症发生率为0,明显低于A组的31.25%、C组的33.34%,差异有统计学意义(P<0.05)。结论子宫颈高级别上皮内瘤变患者宫颈锥切术后间隔4~6周再行腹腔镜下全子宫切除术,术后发生感染、其他并发症的风险较低,手术治疗效果最佳。Objective To exploring the interval between laparoscopic total hysterectomy after cervical conization in patients with high-grade intraepithelial neoplasia of the cervix.Methods 46 patients with high-grade cervical intraepithelial neoplasia who underwent laparoscopic total hysterectomy after cervical conization in our hospital were selected as the research subjects.They were divided into three groups based on the interval between laparoscopic total hysterectomy after cervical conization.Group A(16 cases)had an interval of 72 hours,group B(15 cases)had an interval of 4-6 weeks,and group C(15 cases)had an interval of 6-8 weeks.The surgical indicators and postoperative complications of the three groups were compared.Results There was a statistically significant difference in surgical time,intraoperative bleeding,postoperative white blood cell count,and hospital stay among the three groups(P<0.05),and group B was significantly better than groups A and C(P<0.05);The incidence of postoperative complications in group B was 0,significantly lower than 31.25%in group A and 33.34%in group C,with a statistically significant difference(P<0.05).Conclusion Laparoscopic total hysterectomy is performed every 4-6 weeks after cervical cone resection in patients with high-grade intraepithelial neoplasia of the cervix,with a lower risk of postoperative infection and other complications,and the best surgical treatment effect.
