宫颈活检组织病理学与液基薄层细胞学检查诊断宫颈上皮内瘤变的临床意义

摘　　要：目的探讨在宫颈上皮内瘤变患者诊断中宫颈活检组织病理学和液基薄层细胞学检查的临床价值。方法选取106例疑似宫颈上皮内瘤变患者,患者均进行宫颈活检组织病理学检查和液基薄层细胞学检查,以宫颈锥切术病理检查结果作为金标准,观察患者使用宫颈活检组织病理学检查和液基薄层细胞学检查的诊断效能(敏感度、特异度和诊断准确率)。结果宫颈锥切术病理检查结果显示,有100例阳性患者,有6例阴性患者。宫颈活检组织病理学检查结果显示,有99例阳性患者,有7例阴性患者,其敏感度为98.00%,特异度为83.33%,诊断准确率为97.17%。液基薄层细胞学检查结果显示,有95例阳性患者,有11例阴性患者,其敏感度为91.00%,特异度为33.33%,诊断准确率为87.74%。宫颈活检组织病理学的敏感度、特异度和诊断准确率均高于液基薄层细胞学检查(P<0.05)。结论在宫颈上皮内瘤变患者的诊断中,与液基薄层细胞学检查相比,宫颈活检组织病理学检查的敏感度、特异度和诊断准确率均较高。Objective To investigate the clinical value of cervical biopsy histopathology and liquid-based thin layer cytology in the diagnosis of cervical biopsy histopathology.Methods 106 patients with suspected cervical intraepithelial neoplasia were selected and all of them underwent cervical biopsy histopathology and liquid-based thin layer cytology.The diagnostic efficacy(sensitivity,specificity and diagnostic accuracy)of cervical biopsy histopathology and liquid-based thin layer histopathology were observed with the results of cervical conectomy pathology as the gold standard.Results The pathological examination results of cervical conectomy showed that 100 patients were positive and 6 were negative.The results of cervical biopsy histopathology showed that there were 99 positive patients and 7 negative patients,the sensitivity was 98.00%,the specificity was 83.33%,and the diagnostic accuracy was 97.17%.There were 95 positive patients and 11 negative patients,the sensitivity was 91.00%,the specificity was 33.33%,and the diagnostic accuracy was 87.74%.The sensitivity,specificity and diagnostic accuracy of cervical biopsy were higher than those of liquid-based thin layer cytology(P<0.05).Conclusion In the diagnosis of cervical intraepithelial neoplasia,the sensitivity,specificity and diagnostic accuracy of cervical biopsy histopathology are higher than that of liquid-based thin layer cytology.
