速溶胃肠超声联合胃镜对胃内癌变的诊断价值

摘　　要：目的探讨联合应用速溶胃肠超声及胃镜对胃内癌变的诊断价值。方法选取2019年9月至2022年5月收治的200例疑似胃癌患者作为研究对象,均行速溶胃肠超声检查、胃镜检查及手术病理检查,其中以手术病理检查为金标准,分析联合使用速溶胃肠超声与胃镜检查对胃癌的诊断效能。结果200例疑似胃癌患者中有188例经手术病理确诊,其中包括胃印戒细胞癌24例,腺癌164例;应用速溶胃肠超声诊断的阳性预测值为96.67%、阴性预测值为30.00%;应用胃镜检查的阳性预测值为96.43%、阴性预测值为18.75%;速溶胃肠超声诊断与胃镜联合诊断的阳性预测值为98.94%、阴性预测值为100%;联合检查的特异度、敏感性及准确度分别为100%、98.94%、99%,均明显高于单一检查(P<0.05)。结论术前胃癌患者的临床诊断可采用速溶胃肠超声检查方法,此方法具有较高的阳性预测值,在胃癌患者的全面诊断时应联合胃镜检查,可显著提高胃癌的诊断效能。Objective To explore the application value of instant gastrointestinal ultrasound combined with gastroscopy in the diagnosis of gastric cancer.Methods A total of 200 patients with suspected gastric cancer who were admitted to our hospital from September 2019 to May 2022 were selected as the research objects,and all of them underwent gastrointestinal ultrasound with instant contrast agents,gastroscopy and postoperative pathological examination.With the postoperative pathology as the gold standard,the diagnostic efficacy of instant gastrointestinal ultrasound combined with gastroscopy was assessed.Results A total of 188/200 patients were finally diagnosed as gastric cancer by postoperative pathology,including 24 cases of gastric signet-ring gastric carcinomas and 164 cases of adenocarcinomas.The positive and negative predictive values of instant gastrointestinal ultrasound in diagnosing gastric cancer were 96.67%,and 30.00%,respectively,which,of gastroscopy were 96.43%and 18.75%,respectively.The positive and negative predictive values of instant gastrointestinal ultrasound combined with gastroscopy in diagnosing gastric cancer were 98.94%and 100%,respectively.The specificity,sensitivity and accuracy of the combined test were 100%,98.94%and 99%,respectively,all of which were significantly higher than those of a single detection(P<0.05).Conclusion Instant gastrointestinal ultrasound can be applied to the preoperative diagnosis of gastric cancer,presenting a higher positive predictive value.Its combination with gastroscopy significantly enhances the diagnostic potential of gastric cancer.
