摘  要:目的探讨血清C-X3-C基序趋化因子配体1(CX3CL1)联合视黄醇结合蛋白4(RBP4)对急性脑梗死(ACI)患者静脉溶栓预后的评估作用。方法选取2020-01—2022-06内蒙古医科大学附属医院收治的120例ACI患者为ACI组,根据改良Rankin量表分为预后不良组和预后良好组,另选取同期60名体检健康志愿者为对照组。采用酶联免疫吸附法检测血清CX3CL1、RBP4水平。多因素Logistic回归和受试者工作特征曲线分析血清CX3CL1、RBP4水平对ACI患者静脉溶栓后预后不良的影响。结果与对照组比较,ACI组血清CX3CL1、RBP4水平升高(P<0.05)。随访3个月,120例ACI患者预后不良发生率为35.00%(42/120)。年龄、NIHSS评分、CX3CL1、RBP4为ACI患者静脉溶栓后预后不良的独立危险因素,弥散加权成像-阿尔伯塔卒中项目早期计算机断层扫描评分为独立保护因素(P<0.05)。血清CX3CL1、RBP4和二项联合评估预后不良的曲线下面积分别为0.779、0.778、0.876,二项联合的曲线下面积最大。结论ACI患者血清CX3CL1、RBP4水平升高,与静脉溶栓后预后不良独立相关且预测价值较高,可能成为ACI患者静脉溶栓后预后的辅助评估指标。Objective To investigate the value of serum C-X3-C motif chemokine ligand 1(CX3CL1)combined with retinol-binding protein 4(RBP4)in assessing the prognosis of patients with acute cerebral infarction(ACI)after intravenous thrombolysis.Methods The 120 patients with ACI admitted to the Affiliated Hospital of Inner Mongolia Medical University from January 2020 to June 2022 were selected as the ACI group and were divided into poor prognosis group and good prognosis group according to the modified Rankin scale,and another 60 healthy volunteers with physical examination in the same period were selected as the control group.Serum CX3CL1 and RBP4 levels were measured by ELISA.The factors influencing poor prognosis after intravenous thrombolysis in patients with ACI were analyzed,and the value of serum CX3CL1 and RBP4 levels on poor prognosis after intravenous thrombolysis in patients with ACI was assessed using receiver operating characteristic curves.Results Serum CX3CL1 and RBP4 levels were increased in the ACI group compared to the control group(P<0.05).At 3 months follow-up,the incidence of poor prognosis in 120 ACI p35.00%(42/120). Age,National Institutes of Health stroke scale score,CX3CL1,and RBP4 were independentrisk factors for poor prognosis after intravenous thrombolysis in patients with ACI,and diffusion-weightedimaging-Alberta stroke project early computed tomography score was an independent protective factor(P<0.05).The area under curve for serum CX3CL1,RBP4 and the combination of the two items to assess poor prognosisafter intravenous thrombolysis in patients with ACI was 0.779,0.778 and 0.876,respectively,with the largestarea under curve for the combination of the two items. Conclusion Elevated serum CX3CL1 and RBP4 levels inACI patients are independently associated with poor prognosis after intravenous thrombolysis,and CX3CL1combined with RBP4 is of high value in assessing poor prognosis after intravenous thrombolysis in ACI patients,and may be an adjunctive assessment index for prognosis after intravenous
