摘  要:目的探究八段锦联合肌电生物反馈治疗对脑卒中偏瘫患者肌电图特征以及自我动态平衡的影响。方法选取2020-06—2022-06在南京医科大学附属南京医院接受康复治疗的100例脑卒中偏瘫患者,采用随机数字表法分成常规组和研究组,每组50例。常规组采用肌电生物反馈治疗,研究组在常规组的基础上给予八段锦治疗,对比2组患者自我动态平衡、肌电图特征、运动功能[Fugl-Meyer运功功能评分量表(FMA)评分]以及自理能力[Barthel指数评定量表(BI)评分]情况。结果治疗后,研究组患者站立位的重心单位面积移动轨迹长度、重心移动轨迹总长度和重心摆动平均速度均小于常规组(P<0.05)。研究组患者治疗后的腓肠肌、股四头肌和胫骨前肌iEMG均高于常规组(P<0.05),腓肠肌、股四头肌和胫骨前肌RMS均低于常规组(P<0.05),上下肢FMA评分均高于常规组(P<0.05)。研究组患者治疗4、8周后BI评分均高于常规组(P<0.05)。结论八段锦联合肌电生物反馈能够提高脑卒中偏瘫患者自我动态平衡能力,改善患者肌肉功能和运动功能,提高其自理能力。Objective To investgate the impacts of Baduanjin combined with EMG biofeedback therapy on EMG characteristics and self-dynamic balance of stroke hemiplegic patients.Methods One hundred stroke hemiplegic patients who received rehabilitation treatment in Nanjing First Hospital,Nanjing Medical University from June 2020 to June 2022 were selected,with 50 patients in each group.The routine group was treated by EMG biofeedback therapy,while the study group was treated by Baduanjin on the basis of the routine group.The self-dynamic balance,EMG characteristics,motor function[Fugl-Meyer assessment(FMA)score]and self-care ability[Barthel index(BI)score]were compared between the two groups.Results After treatment,the length of the movement trajectory of the center of gravity per unit area,the total length of the center of gravity movement trajectory and the average speed of the center of gravity swing in the standing position of the patients in the study group were lower than those in the conventional group(P<0.05).The iEMG of gastrocnemius,quadriceps and tibial anterior muscle in the study group were higher than those in the conventional group(P<0.05).The RMS of gastrocnemius,quadriceps and tibial anterior muscle in the study group were all lower than those in the conventional group(P<0.05).The FMA scores of upper and lower extremities were higher than those in the conventional group(P<0.05).The BI scores of the study group after 4 and 8 weeks of treatment were higher than those in the conventional group(P<0.05).Conclusion Baduanjin combined with EMG biofeedback can improve the self-dynamic balance ability of stroke hemiplegic patients,improve their muscle function and motor function,and improve their self-care ability.
