急性缺血性卒中患者救治时间延迟的现状

摘　　要：脑血管疾病十分凶险,危及生命安全,尤其是脑卒中,近年来发病率逐渐提高,急性缺血性卒中作为其中常见类型,是由各种原因所导致,患者常见脑血管栓塞,进而导致相应供血区或脑细胞出现缺血、缺氧、坏死现象,出现各种功能障碍。该疾病的治疗在于尽早恢复脑组织血液再灌注,尽可能挽救缺血半暗带,帮助其恢复正常血运,减少对神经功能的损伤。溶栓作为此类疾病治疗的重要手段,然而溶栓对时间窗有着严格要求,患者的黄金救治时间是发病4.5 h之内,尤其是发病3 h之内为溶栓的最佳时间,然而受多方面因素影响,许多患者常错失最佳溶栓时间,而超过时间窗再采取溶栓治疗不仅无法取得良好的治疗效果,甚至对患者产生极大危害。本文通过对急性缺血性卒中患者救治时间延迟的现状及影响因素进行综述,望为今后建立系统且规范的卒中救治模式提供参考。Cerebrovascular diseases are very dangerous and endanger life safety,especially stroke.In recent years,the incidence rate has gradually increased.Acute ischemic stroke,as one of the common types,causes common cerebrovascular occlusion in patients,which in turn leads to ischemia,hypoxia,necrosis and various functional disorders in corresponding blood supply areas or brain cells.The treatment of the disease lies in restoring cerebral tissue blood reperfusion as soon as possible,saving ischemic penumbra as much as possible,helping it restore normal blood supply and reducing the damage to nerve function.Thrombolysis is an important method for the treatment of this kind of diseases,but it has strict requirements on the time window.The golden treatment time for patients is within 4.5 h hours of onset,especially within 3 hours of onset.However,due to many factors,many patients often miss the best thrombolytic time,and taking thrombolytic treatment beyond the time window can not only achieve good therapeutic effect,but even cause great harm to patients.This paper comprehensively analyzes the current situation and influencing factors of the delay in treatment of patients with acute ischemic stroke,hoping to provide reference guidance for establishing a systematic and standardized stroke treatment model in the future.
