基于“补气活血”理论探讨胃癌前病变的“炎癌转化”

摘　　要：胃癌前病变(PLGC)是以胃黏膜萎缩、固有腺体减少为主要病理特征的炎性疾病,控制PLGC炎癌转化对于预防胃癌具有重要意义。PLGC包括胃黏膜萎缩、肠上皮化生及不典型增生,具有病程日久、缠绵反复的特点。纵观各医家之见解,笔者认为PLGC以脾胃气虚为本,痰凝血瘀为标。同时提出PLGC治疗应以补气活血为基本大法,既顾护脾胃之气,又祛除胃部邪气,从而达到改善胃黏膜内环境,增强机体免疫力,控制炎症向癌症转化的治疗效果。以补气活血为视角阐述PLGC炎癌转化,注重个体化差异,辨证论治,从中医学角度阐述PLGC的科学内涵,为预防及治疗胃癌提供理论依据。Gastric precancerous lesion(PLGC)is an inflammatory disease characterized by atrophy of gastric mucosa and reduction of inherent glands.Controlling PLGC inflammation and cancer transformation is of great significance for preventing gastric cancer.PLGC is commonly characterized by gastric mucosal atrophy,intestinal metaplasia and atypical hyperplasia,with a long course and recurrent entanglement.Looking at the opinions of various medical experts,the author believes that PLGC is based on spleen and stomach qi deficiency,with phlegm coagulation and blood stasis as the standard.Treating on PLGC should be based on the basic principle of nourishing qi and promoting blood circulation,which not only protects the qi of spleen and stomach,but also eliminates the evil qi in the stomach,in order to improve the internal environment of gastric mucosa,enhance the body’s immunity,and control the transformation of inflammation to cancer.From the perspective of invigorating qi and activating blood circulation,this article elaborates on the inflammatory and cancerous transformation of PLGC,emphasizing individual differences,differentiation of symptoms and treatment,and elaborates on the scientific connotation of PLGC from the perspective of traditional Chinese medicine,providing theoretical basis for the prevention and treatment of gastric cancer.
