摘　　要：目的探讨姑息治疗肺癌患者症状群的组成及其对生活质量的影响。方法选取医院2020年8月至2021年3月收治的姑息治疗肺癌患者100例,治疗3个月。采用自制一般情况调查表收集一般资料,采用肺癌患者生存质量测定(FACT-L)量表和安德森症状评估量表(MDASI-C)调查症状严重程度和生活困扰程度,采用世界卫生组织生存质量测定简表(WHOQOL-BREF)从生理维度、心理维度、环境维度、社会维度评估患者治疗前后的生活质量。结果姑息治疗肺癌患者最终确定6个症状群,包括肺癌特异性症状组、胃肠道症状组、疲劳相关症状组、神经相关症状组、心理症状组、感知症状组。治疗后,WHOQOL-BREF各维度评分均显著高于治疗前(P<0.05)。结论姑息治疗可有效缓解患者的临床症状,改善营养水平和生理状况,提高心理能力、社会能力及生活质量。Objective To investigate the composition of symptom groups of patients with lung cancer treated with palliative care and its effect on quality of life.Methods A total of 100 lung cancer patients receiving palliative care admitted to the hospital from August 2020 to March 2021 were selected.Both groups were treated for three months.A self-made general situation questionnaire was used to collect general information,and the functional assessment of cancer therapy-lung cancer(FACT-L)and M.D.Anderson Symptom Inventory(MDASI-C)were used to investigate the severity of symptoms and life distress.The World Health Organization Quality of Life-BREF(WHOQOL-BREF)was used to assess the quality of life of patients before and after treatment from the physiological,psychological,environmental and social dimensions.Results Six symptom groups were ultimately identified in palliative care for lung cancer patients,including lung cancer specific symptom group,gastrointestinal symptom group,fatigue-related symptom group,neurological-related symptom group,psychological symptom group,and perceptual symptom group.After treatment,the WHOQOL-BREF scores in all dimensions were significantly higher than those before treatment(P<0.05).Conclusion Palliative care can effectively alleviate clinical symptoms,improve nutritional and physiological conditions,and improve psychological ability,social ability,and quality of life of patients.
