摘  要:目的探讨糖类抗原19-9(CA19-9)、胰岛素样生长因子结合蛋白3(IGFBP3)、巨噬细胞抑制因子-1(MIC-1)检测对胰腺癌患者预后的预测价值。方法选取82例胰腺癌患者作为观察组,另选取81例健康体检志愿者作为对照组。对比两组受试者MIC-1、IGFBP3、CA19-9水平,分析胰腺癌患者预后的影响因素和MIC-1、IGFBP3、CA19-9单独及联合检测对胰腺癌患者预后的预测价值。结果观察组患者MIC-1、IGFBP3、CA19-9水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。82例胰腺癌患者根据预后情况分为好转组61例与预后不良组21例,好转组与预后不良组患者分化程度、临床分期、淋巴结转移情况及MIC-1、IGFBP3、CA19-9水平比较,差异均有统计学意义(P﹤0.05)。分化程度为低分化、临床分期为Ⅲ~Ⅳ期、有淋巴结转移、MIC-1≥500 pg/ml、IGFBP3≥40 U/ml、CA19-9≥40 U/ml均为胰腺癌患者预后不良的独立危险因素(P﹤0.05)。MIC-1、IGFBP3、CA19-9联合检测预测胰腺癌患者预后的灵敏度和特异度分别为87.4%、71.9%,曲线下面积为0.816(95%CI:0.715~0.917),高于MIC-1、IGFBP3、CA19-9单独检测。结论MIC-1、IGFBP3、CA19-9水平升高均与胰腺癌患者预后不良密切相关,三者在胰腺癌患者预后预测中有较高的使用价值。Objective To analyze the predicting value of carbohydrate antigen 19-9(CA19-9),insulin like growth factor binding protein 3(IGFBP3),macrophage inhibitory cytokine-1(MIC-1)in the prognosis of patients with pancreatic cancer.Method A total of 82 patients with pancreatic cancer were selected into the observation group,and 81 healthy volunteers during the same period into the control group.The levels of MIC-1,IGFBP3,and CA19-9 in two groups were compared,the risk factors for prognosis and the predictive value of MIC-1,IGFBP3,and CA19-9 single and combined detection in pancreatic cancer patients were analyzed.Result The levels of MIC-1,IGFBP3 and CA19-9 in the observation group were significantly higher than those in the control group(P<0.01);82 cases of pancreatic cancer were divided into the improved group with 61 cases and poor prognosis group with 21 cases according to the prognosis.There were differences in differentiation degree,clinical stage,lymphatic metastasis and levels of MIC-1,IGFBP3,and CA19-9 in improved group versus poor prognosis group(P<0.05).Low differentiation,clinical stage III-IV,lymphatic metastasis,MIC-1≥500 pg/ml,IGFBP3≥40 U/ml and CA19-9≥40 U/ml were independent risk factors for poor prognosis of patients with pancreatic cancer(P<0.05).The sensitivity and specificity of MIC-1,IGFBP3 and CA19-9 combined detection for predicting prognosis of patients with pancreatic cancer were 87.4%and 71.9%,respectively,and area under the curve(AUC)was 0.816(95%CI:0.715-0.917),which was significantly higher than single detection of MIC-1,IGFBP3 and CA19-9.Conclusion The increased levels of MIC-1,IGFBP3,and CA19-9 are closely related to the poor prognosis of patients with pancreatic cancer,which have high value in predicting the prognosis of patients with pancreatic cancer.
