摘 要:目的探讨血清胸苷激酶1(TK1)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)及dickkopf WNT信号通路抑制剂1(DKK1)对食管癌的诊断价值。方法选取62例食管癌患者作为食管癌组,59例健康体检者作为对照组。对比两组受试者TK1、CYFRA21-1、DKK1水平,对比不同分期食管癌患者TK1、CYFRA21-1、DKK1水平,分析TK1、CYFRA21-1、DKK1单独及联合检测对食管癌的诊断价值。结果食管癌组患者TK1、CYFRA21-1、DKK1水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。Ⅲ~Ⅳ期食管癌患者TK1、CYFRA21-1、DKK1水平均明显高于Ⅰ~Ⅱ期患者,差异均有统计学意义(P﹤0.01)。TK1+CYFRA21-1+DKK1联合检测诊断食管癌的灵敏度和特异度分别为88.50%、79.10%,曲线下面积为0.779(95%CI:0.695~0.864),均高于三者单独检测。结论TK1、CYFRA21-1、DKK1联合检测有利于提高食管癌的诊断效能,防止误诊漏诊,三者对其病情评估具有一定参考价值。Objective To investigate the diagnostic value of thymidine kinase 1(TK1),cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)and dickkopf WNT signaling pathway inhibitor 1(DKK1)in esophageal cancer.Method A total of 62 patients with esophageal cancer were selected into the esophageal cancer group,and 59 healthy volunteers into the control group.The levels of TK1,CYFRA21-1 and DKK1 in two groups and in esophageal cancer patients with different stages were compared,and the diagnostic value of single and combined detection of TK1,CYFRA21-1 and DKK1 in esophageal cancer were analyzed.Result The levels of TK1,CYFRA21-1 and DKK1 in esophageal cancer group were higher than those in control group(P<0.01);the levels of TK1,CYFRA21-1 and DKK1 in stage Ⅲ-Ⅳ patients were significantly higher than those in stage Ⅰ-Ⅱ(P<0.01).The sensitivity and specificity of TK1+CYFRA21-1+DKK1 detection for the diagnosis of esophageal cancer were 88.50% and 79.10%,respectively,and the area under the curve(AUC)was 0.779(95%CI:0.695-0.864),which were significantly higher than those of the single detection.Conclusion The combined detection of TK1,CYFRA21-1 and DKK1 can improve the diagnostic efficiency of esophageal cancer and prevent misdiagnosis and missed diagnosis,which can provide reference for the disease evaluation of patients.
