摘 要:胰腺癌患病率逐年增长,患者5年生存率低于5%。在各种胰腺癌中,胰腺导管腺癌(Pancreatic ductal adenocarcinoma, PDAC)约占90%。PDAC是一种缺乏血液供应的肿瘤,在缺氧诱导因子-1α (Hypoxia inducible factor-1α,HIF-1α)的调节下,即使在缺氧环境,肿瘤细胞也能存活。胰腺上皮内瘤变(Pancreatic intraepithelial neoplasia, PanIN)是PDAC的主要癌前病变。基于形态学异常程度,PanIN分为PanIN-1、PanIN-2、PanIN-3 3级,其中PanIN-3又称原位癌。PanIN与PDAC有相同的基因异常谱。HIF-1α作为缺氧环境中关键的调节因子,对PanIN、PDAC相关基因起到显著的调节作用。由于缺乏合适的研究工具,HIF-1α在胰腺导管腺癌的形成和进展过程中的变化没有得到充分的研究和解释。本文根据PDAC发生发展的不同阶段,讨论HIF-1α对癌前病变PanIN及PDAC相关基因的影响。The prevalence of pancreatic cancer is increasing year by year,and the 5-year survival rate of patients is less than 5%.Among the various pancreatic cancers,pancreatic ductal adenocarcinoma(PDAC)accounts for about 90%.PDAC is a tumor that lacks blood supply and can survive even in hypoxic environments when regulated by hypoxia inducible factor-1α(HIF-1α).Pancreatic intraepithelial neoplasia(PanIN)is the precancerous lesion of PDAC.Based on the degree of morphological abnormality,PanIN is divided into three grades:PanIN-1,PanIN-2 and PanIN-3,of which PanIN-3 is also known as carcinoma in situ.PanIN shows the same genetic abnormality profile as PDAC.As a key regulator in hypoxic environment,HIF-1αplays a significant regulatory role in PanIN and PDAC-related genes.Due to the lack of suitable research tools,the changes of HIF-1αin the formation and evolution of PDAC have not been adequately studied and explained.This article discusses the effects of HIF-1αon PanIN and PDAC-related genes according to the different stages in occurrence and development of PDAC.
