摘 要:传播与不确定性管理理论(TCUM)重点关注不确定性体验、健康决策的方式与意义,该研究以乳腺癌医患沟通为具体研究对象,将“时间-情感立场”作为一个关联因素引入该理论。研究发现:“时间”引发的“情感立场”会对医患不确定性认知、评价和决策产生差异,从而阻碍医患顺畅沟通。这样的发现也从实践层面提出了理解和消除“情感立场”差异对改善沟通、促进医患关系的现实路径。Based on Uncertainty Management Theory,this study examines time-emotional stance using breast cancer as a typical example.Deeply rooted in the Chinese context,this study acknowledges doctors as information providers in a patient's uncertainty management(cognition,evaluation,and decisionmaking).This two-year study was conducted with 42 patient-doctor interviews,8 online observation groups,and 11 public welfare keynote speeches.Through emotional computing and discourse analysis,the study made several conclusions:Emotional stance triggered by time will differentiate doctor-patient uncertainty cognition,evaluation,and decision-making.Smooth doctor-patient communication is thus impeded.This study proposes practical ways to improve doctor-patient communication by understanding emotional stance differences.Their relationship is also promoted in this sense.Meanwhile,this study verifies and extrapolates emotional stance as a prominent factor to expand the Uncertainty Management Theory of Communication.The guiding value of this localized theory for further studies of health relationship communication in China is also explored.
