乳果糖口服溶液与门冬氨酸鸟氨酸联治对肝性脑病患者神经毒性物质、炎症的影响

摘　　要：目的探究乳果糖口服溶液与门冬氨酸鸟氨酸联治对肝性脑病患者神经毒性物质、炎症的影响。方法选取郑州大学第一附属医院2020年1月至2022年6月接受肝性脑病治疗的40例患者纳入本次研究,将所有患者随机分为观察组和对照组,每组20例。对照组给予乳果糖口服溶液进行治疗,观察组则通过乳果糖口服溶液联合门冬氨酸鸟氨酸进行联合治疗。结果治疗后观察组的临床治疗总有效率(95.0%)高于对照组(70.0%),组间比较差异有统计学意义(P<0.05);观察组治疗后的肝功能各项指标比对照组下降更为显著(P<0.05);两组患者治疗后的血氨水平以及β-内啡肽等毒性物质较之于治疗前均有所下降,观察组较之于对照组下降更明显(P<0.05);观察组治疗后的各项炎症指标水平明显低于对照组(P<0.05);两组患者尿、血淀粉酶(AMY)水平均有明显的下降,而观察组较之于对照组下降更为显著(P<0.05);治疗后观察组肝性脑病积分明显低于对照组(P<0.05)。结论门冬氨酸鸟氨酸与乳果糖口服溶液联合治疗肝性脑病,有显著的临床疗效,能够降低炎症因子和神经毒性物质水平,值得推广。Objective To explore the effects of lactulose oral solution combined with ornithine aspartate on neurotoxic sub⁃stances and inflammation in patients with hepatic encephalopathy.Methods A total of 40 patients who received treatment for hepatic encephalopathy in the First Affiliated Hospital of Zhengzhou University from January 2020 to June 2022 were in⁃cluded in this study,and all the patients were randomly divided into observation group and control group,with 20 cases in each group.The control group was treated with lactulose oral solution alone,while the observation group was treated with lactulose oral solution plus ornithine aspartate.Results After treatment,the total effective rate of observation group(95.0%)was higher than that of control group(70.0%),and there was statistical significance between groups(P<0.05).The indexes of liver function in the observation group after treatment were more significantly decreased than those in the control group(P<0.05).After treatment,blood ammonia level and toxic substances such asβ-endorphin in two groups were decreased compared with before treatment,and the decrease was more obvious in observation group compared with control group(P<0.05).The levels of various inflammatory indexes in the observation group were signifi⁃cantly lower than those in the control group after treatment(P<0.05).Urine and blood AMY levels in two groups were significantly decreased,and the decrease in observation group was more significant than that in control group(P<0.05).The score of hepatic encephalopathy in the observation group was significantly lower than that of the control group after treatment(P<0.05).Conclusion The combination of ornithine aspartate and lactulose oral solution in the treatment of hepatic encephalopathy has significant clinical efficacy,can reduce the levels of inflammatory factors and neurotoxic sub⁃stances,and is worth promoting.
