摘 要:目的探讨应用双源CT双能量碘图来无创评价肝硬化门静脉高压患者的门静脉血流动力学变化。方法对2019年12月—2021年12月期间河南中医药大学第一附属医院门脉高压组(肝硬化患者50例)、正常对照组(22例正常人)均采用双能扫描序列进行双期增强扫描,测量肝脾动脉期、静脉期碘浓度及门静脉主干碘浓度。结果①门静脉高压组肝脏动脉期碘浓度(0.74±0.22)mgI/mL较对照组(0.50±0.12)mgI/mL高(t=6.092,P<0.05);门静脉高压组肝脏静脉期碘浓度(1.87±0.26)mgI/mL较对照组(2.14±0.32)mgI/mL低(t=3.825,P<0.05);门静脉高压组脾脏动脉期碘浓度(3.79±1.16)mgI/mL较对照组(3.03±0.82)mgI/mL高(t=2.728,P<0.05);门静脉高压组脾脏静脉期碘浓度(2.66±0.47)mgI/mL与对照组(2.58±0.63)mgI/mL相比无统计学差异(t=0.580,P>0.05);门静脉高压组门静脉主干碘浓度(4.11±0.65)mgI/mL高于对照组(5.35±0.42)mgI/mL(t=9.551,P>0.05)。②肝脏动脉期碘浓度、脾脏动脉期碘浓度与肝硬化门静脉高压有正相关关系(r=0.554、0.301,P<0.05);肝脏静脉期碘浓度和门静脉主干碘浓度与肝硬化门静脉高压有负相关关系(r=-0.395、-0.738,P<0.05),脾脏门静脉期碘浓度与肝硬化门静脉高压则无关(r=-0.015,P>0.05),其中门静脉主干碘浓度相关性高。结论门静脉主干碘浓度与肝硬化门静脉高压有较好的相关性,可以对门静脉高压患者门静脉系统血流变化情况进行无创性评价;双能碘图门静脉主干碘浓度可用于评价肝硬化患者门静脉高压症。Objective To explore the use of dual-source CT dual-energy iodine map to non-invasive evaluate the changes of portal vein hemodynamics in patients with portal hypertension in cirrhosis.Methods From December 2019 to December 2021,Both portal hypertension group(50 patients with cirrhosis)and normal control group(22 normal people)were scanned with dual-energy scanning sequence to measure the iodine concentration in the arterial and venous phases of the liver and spleen and in the main portal vein.Results①The iodine concentration in the hepatic artery phase of portal hypertension group(0.74±0.22)mgI/mL was higher than that of the control group(0.50±0.12)mgI/mL(t=6.092,P<0.05);The iodine concentration in the hepatic venous phase of portal hypertension group(1.87±0.26)mg/mL was lower than that of the control group(2.14±0.32)mg/mL(t=3.825,P<0.05);The iodine concentration in splenic ar⁃tery phase(3.79±1.16)mg/mL in portal hypertension group was higher than that of control group(3.03±0.82)mg/mL(t=2.728,P<0.05);There was no significant difference in iodine concentration in splenic venous phase between portal hypertension group(2.66±0.47)mgI/mL and control group(2.58±0.63)mgI/mL(t=0.580,P>0.05);The concen⁃tration of iodine in the main portal vein of portal hypertension group(4.11±0.65)mgI/mL was higher than that of the control group(5.35±0.42)mgI/mL(t=9.551,P>0.05).②There was a positive correlation between the iodine con⁃centration in hepatic artery phase and splenic artery phase and portal hypertension in cirrhosis(r=0.554,0.301,P<0.05);There was a negative correlation between the concentration of iodine in the hepatic venous phase and in the main portal vein and the portal hypertension in cirrhosis(r=-0.395,-0.738,P<0.05),but there was no correlation between the concentration of iodine in the splenic portal vein phase and the portal hypertension in cirrhosis(r=-0.015,P>0.05),among which the concentration of iodine in the main portal vein was highly correlated.Conclusion There is a good correlation betw
