牙周炎伴2型糖尿病患者戴用可摘局部义齿的效果研究

摘　　要：目的:探讨老年中重度牙周炎伴2型糖尿病患者戴用可摘局部义齿后牙周状况。方法:选取佳木斯市口腔病防治院口腔内科2020年8月—2021年8月诊疗的96例老年中重度牙周炎伴2型糖尿病患者为对象,应用随机摸球法分为对照组和研究组,每组48例。对照组予以常规牙周炎治疗,研究组予以可摘局部义齿修复治疗,比较两组糖代谢水平、牙周指标、牙齿松动度、MMP-8水平。结果:两组戴用义齿1、3个月后GI、PLI、PD水平高于修复前,且研究组高于对照组(P<0.05);研究组戴用义齿1、2、3个月牙齿松动度大于对照组(P<0.05);研究组戴用义齿1、2、3个月MMP-8水平高于对照组(P<0.05);研究组戴用义齿1、3个月后空腹血糖、糖化血红蛋白水平低于对照组(P<0.05)。结论:老年中重度牙周炎伴2型糖尿病患者佩戴可摘局部义齿后的牙龈炎症较为严重,可导致MMP-8水平升高,患者需注意加强口腔卫生干预,有效减少义齿对于基牙、其他残余牙周的损伤,指导定期复查,加强牙周状况的监测,有效改善老年患者生活质量。Objective:To explore the periodontal status of elderly patients with moderate and severe periodontitis and type 2 diabetes after wearing removable partial dentures.Methods:Ninety-six elderly patients with moderate and severe periodontitis and type 2 diabetes treated in the Department of Stomatology,Jiamusi Stomatology Prevention and Treatment Hospital from August 2020 to August 2021 were selected as subjects,and randomly divided into control group and research group with 48 cases in each group.The control group was treated with conventional periodontitis,while the research group was treated with removable partial denture.The levels of glucose metabolism,periodontal index,tooth looseness and MMP-8 were compared between the two groups.Results:The levels of GI,PLI and PD in the two groups were higher than those before restoration after 1 and 3 months of denture wearing,and those in the study group were higher than those in the control group(P<0.05).The loosening degree of teeth in the study group was higher than that in the control group at 1,2 and 3 months(P<0.05).The level of MMP-8 in the study group was higher than that in the control group at 1,2 and 3 months(P<0.05).The levels of fasting blood glucose and glycosylated hemoglobin in the study group were lower than those in the control group at 1 and 3 months after wearing dentures(P<0.05).Conclusion:Gingival inflammation in elderly patients with moderate and severe periodontitis and type 2 diabetes mellitus after wearing removable partial dentures is serious,which can lead to the increase of MMP-8 level.Patients should pay attention to oral hygiene.
