摘　　要：目的探讨乳腺浸润性微乳头状癌患者的临床病理特征以及生存情况。方法回顾性分析1998—2012年山西省人民医院收治的325例乳腺浸润性微乳头状癌患者的病历资料和随访资料。325例患者分为纯合型微乳头状癌及微乳头状癌伴浸润性导管癌(IDC)2组进行比较分析。定性资料采用χ^(2)检验,生存分析采用Kaplan-Meier方法和Cox回归分析。结果单因素分析中,T分期为3期、淋巴结转移(N分期)分期为2~3、雌激素受体(ER)阳性、细胞增殖核抗原(Ki-67)表达为无病生存(DFS)的相关因素(P<0.05),多因素分析显示,N分期、Ki-67为DFS预后相关因素(P<0.05)。总生存(OS)单因素Cox回归分析显示,肿块大小、淋巴结及ER阳性影响总生存(P<0.05),多因素Cox回归分析显示,年龄和淋巴结转移(N分期)为OS的独立危险因素(P<0.05)。结论N分期(N2以上)及高表达Ki-67为浸润性微乳头状癌DFS的独立预后因素,年龄及淋巴结转移分期影响患者OS。Objective To explore the clinicopathological features of patients with invasive micropapillary carcinoma of breast as well as survivals.Methods The medical records and follow-up data of 325 cases of patients from Shanxi Provincial People′s Hospital confirmed by histopathology from 1998 to 2012 were analyzed retrospectively.The cases were categorized into two subgroups by pathology:invasive micropapillary carcinoma and invasive ductal carcinoma.The chi-square was used for categorical variables.The Kaplan-Meier curve and Cox regression was used to evaluate the survival.Results The univariate analysis of DFS indicated that T3,N stage and Ki-67 were significantly associated with DFS.Multivariate analysis of DFS showed N stage Ki-67 were significantly associated with DFS.The univariate analysis indicated that T,N stage and ER were associated with OS,multivariate analysis of OS showed that age and N stage were significantly associated with OS.Conclusions Stage of N≥N2 and Ki-67 were independent risk factors of DFS,age and N stage were independent risk factors of OS.
