多种肿瘤标志物和转氨酶联合检测在肝癌早期诊断和预后评估中的作用

摘　　要：目的探讨肝癌患者诊断与预后评估中,多种肿瘤标志物+转氨酶的检测价值。方法选取2022年4月至2023年5月南阳市第一人民医院收治的82例肝癌患者(肝癌组),选择同期治疗的良性肝病患者82例(良性组),所有入组人员均接受肿瘤标志物与转氨酶检验,比较指标检出情况,统计肝癌复发情况。结果与良性组相比,肝癌组患者的癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原125(CA125)、糖类抗原153(CA153)、糖类抗原199(CA199)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)水平较高(P<0.05);单独检测与联合检测相比,联合检测的敏感度、特异度与符合率均较高(P<0.05);预后评估显示肿瘤标志物与转氨酶联合检测肝癌复发率较高,且复发组的肿瘤标志物与转氨酶水平较高(P<0.05)。结论肿瘤标志物(CEA、AFP、CA125、CA153、CA199)与转氨酶(ALT、AST)联合检测肝癌的准确率较高,且联合检测比单独检测的诊断符合率、预后评估准确率高,值得推广。Objective To explore the value of the detection of multiple tumor markers and transaminases in the diagnosis and prognosis evaluation of liver cancer patients.Methods Eighty-two patients with liver cancer(liver cancer group)in Nanyang First People′s Hospital from April 2022 to May 2023 were randomly selected,and 82 patients with benign liver disease(benign group)who were treated at the same time were selected.All enrolled individuals were tested for tumor markers and transaminases,and the detection of indicators was compared.The recurrence of liver cancer was statistically analyzed.Results Compared with the benign group,the liver cancer group had higher levels of CEA,AFP,CA125,CA153,CA199,ALT,and AST(P<0.05).Compared with combined detection,combined detection has higher sensitivity,specificity,and coincidence rate(P<0.05).The prognosis evaluation showed that the combined detection of tumor markers and transaminases had a higher recurrence rate of liver cancer,and the tumor markers and transaminases levels in the recurrence group were higher(P<0.05).Conclusion The combined detection of tumor markers(CEA,AFP,CA125,CA153,CA199)and transaminases(ALT,AST)for liver cancer has a higher accuracy,and the combined detection has a higher diagnostic accuracy and prognostic evaluation accuracy than the single detection,which is worthy of promoting.
