摘 要:目的在系统文献检索的基础上收集筛选我国大陆地区土壤和人群血硒的数据,并调查了湖北省3540名居民的膳食硒摄入情况,以阐明硒在我国大陆地区土壤和人群血液中的含量分布特征、人群膳食硒摄入量及其对健康的影响。方法从各类文献数据库中系统搜集2002~2022年中国大陆地区各个省份的土壤硒与血硒含量数据,中文文献检索中国知网、万方数据知识服务平台、中国生物医学文献服务系统,英文文献检索Pubmed、web of Science。对文献进行去重和人工筛选、文献梳理和质量评价,排除硒相关非土壤与血硒含量、无法明确按照省份划分的硒含量、动物实验研究及质量差的文献。确定纳入的核心参考文献。整理并加权计算不同省份的土壤和人群血硒含量,对其分布特征进行描述。按照分层多阶段整群随机抽样的原则,调查3054名湖北省居民的每日膳食情况,根据中国食物成分表2009版采用简单分布模型方法,计算膳食硒摄入水平。结果目前我国大陆地区大部分省份土壤处于硒边缘(0.125≤Se<0.175 mg/kg)或硒充足(0.175≤Se<0.4 mg/kg)的状态,大部分省份人群血硒含量接近临床营养状态下限值(100~200μg/L),但土壤硒与人群血硒水平之间的分布趋势不完全一致。膳食调查结果表明,湖北省居民平均每天膳食硒摄入量为18.98μg/d,高端摄入量(P97.5)为69.94μg/d,平均危险商为0.05。结论我国土壤硒和人群血硒水平有明显的地区差异,土壤硒含量和人群血硒水平的相关性不明显,土壤硒含量与居民膳食硒摄入量的相关性也不明显。Objective Based on a systematic literature search,we collected and screened data on soil and population blood selenium in mainland of China and investigated the dietary selenium intake of 3540 residents in Hubei Province to elucidate the distribution characteristics of selenium content in soil and population blood,the dietary selenium intake of the population and its effects on health.Methods The data of soil and blood selenium content of each province in China's Mainland from 2002 to 2022 were systematically collected from various literature databases,Chinese literature was searched on China national knowledge infrastructure,Wanfang Data Knowledge Service Platform,China Biomedical Literature Service,and English literature was searched on Pubmed,web of Science,and the literature was de-weighted and manually screened,sorted and qualitativly evaluated to exclude selenium-related non-soil and blood selenium content,selenium content not clearly classifiable by province,animal experiments and poor study quality.Determine the core references for inclusion.Soil and population blood selenium levels in different provinces were collated and weighted to characterize their distribution.The daily diets of 3054 Hubei residents were investigated according to the principle of stratified multistage whole-group random sampling,and the dietary selenium intake levels were calculated according to the Chinese food composition table 2009 edition using the simple distribution model method.Results At present,the soils in most provinces of China are in the state of marginal selenium(0.125≤Se<0.175 mg/kg)or sufficient selenium(0.175≤Se<0.4 mg/kg),and the population blood selenium levels in most provinces are close to the lower limit of clinical nutritional status(100~200μg/L).But the distribution trends between soil selenium and population blood selenium levels are not completely consistent.The average daily intake of selenium in Hubei province was obtained from the survey as 18.98μg/d,and the high-end intake(P97.5)was 69.94μg/d,wit
