摘　　要：目的 探讨耳穴埋籽联合安神消栓汤加减治疗对卒中后失眠患者血清肿瘤坏死因子-α(TNF-α)水平及睡眠质量的影响。方法 选取2019年3月至2021年3月于沈阳市第二中医院治疗的88例卒中后失眠患者作为研究对象,按随机数字表法分为对照组和观察组,各44例。对照组采用阿普唑仑治疗,观察组采用耳穴埋籽、安神消栓汤加减治疗,两组均治疗1个月。比较两组临床疗效、TNF-α水平、睡眠质量及不良反应。结果 观察组治疗有效率高于对照组,治疗后TNF-α水平高于对照组,匹兹堡睡眠质量指数(PSQI)评分低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 耳穴埋籽、安神消栓汤加减联合治疗卒中后失眠,有利于改善患者睡眠质量,调节血清TNF-α水平,临床应用安全性较高。Objective To explore the effects of auricular point embedding seed combined with Anshenxiaoshuan Decoction on serum tumor necrosis factor-α(TNF-a)level and sleep quality in patients with insomnia after stroke.Methods A total of 88 patients with post-stroke insomnia treated in Shenyang Second Hospital of Traditional Chinese Medicine from March 2019 to March 2021 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 44 cases in each group.The control group was treated with alprazolam,and the observation group was treated with ear point burying seed and Anshen Xiaoshuan decoction.Both groups were treated for 1 month.The clinical efficacy,TNF-a level,sleep quality and adverse reactions were compared between the two groups.Results The effective rate of treatment in observation group was higher than that in control group,the level of TNF-αafter treatment was higher than that in control group,and the score of Pittsburgh Sleep quality index(PSQI)was lower than that in control group,with statistical significance(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combined treatment of seed burying at ear point and Anshen Xiaoshuan Decoction is beneficial to improve the sleep quality of patients and adjust the serum TNF-αlevel,and the clinical application is safe.
