PD-L1在不同组织亚型肺腺癌中表达情况的研究

摘　　要：目的:研究程序性死亡因子配体-1(PD-L1)在不同组织亚型肺腺癌中的表达情况。方法:回顾性分析2021年3月—2023年3月在本院胸外科开展手术切除获得病理标本的肺腺癌患者177例的临床资料,结合不同组织亚型进行分组,后分成贴壁生长型腺癌组40例、腺泡型腺癌组38例、乳头状腺癌组35例、实体型腺癌组32例、微乳头型腺癌组32例,依据HE切片光镜下的形态,采取免疫组化染色法检测肺癌组织中的PD-L1表达情况,比较各组的PD-L1阳性表达率及PD-L1 TPS评分差异。结果:各组肺癌组织中的PD-L1阳性表达率及PD-L1 TPS评分相比差异显著(P<0.05),腺泡型腺癌组肺癌组织中的PD-L1阳性表达率及PD-L1 TPS评分高于贴壁生长型腺癌组(P<0.05),乳头状腺癌组肺癌组织中的PD-L1阳性表达率及PD-L1 TPS评分高于腺泡型腺癌组(P<0.05),微乳头型腺癌组肺癌组织中的PD-L1阳性表达率及PD-L1 TPS评分高于乳头状腺癌组(P<0.05),实体型腺癌组肺癌组织中的PD-L1阳性表达率及PD-L1 TPS评分高于微乳头型腺癌组(P<0.05)。结论:PD-L1在不同组织亚型肺腺癌中的表达情况有所差异,其中实体型肺腺癌患者肿瘤组织中的PD-L1阳性表达水平较高,可能对免疫治疗更加获益。Objective:To study the expression of the programmed death factor ligand-1(PD-L1)in lung adenocarcinoma of different tissue subtypes.Methods:The clinical data of 177 lung adenocarcinoma patients in the thoracic surgery from March 2021 to March 2023 were divided into different tissue subtypes and divided into 40 cases,adherent growth adenocarcinoma,38 cases,32 cases of papillary adenocarcinoma,PD-L1 expression in lung cancer was detected by immunohistochemical staining to compare the positive expression rate of PD-L1 and PD-L1 TPS score of each group.Results:The difference of PD-L1 expression and PD-L1 TPS score in lung cancer tissues(P<0.05),The positive PD-L1 expression rate and PD-L1 TPS score in the acinar adenocarcinoma group were higher than those in the adherent growth adenocarcinoma group(P<0.05),PD-L1 positive expression rate and PD-L1 TPS score in the papillary adenocarcinoma group than the acinar adenocarcinoma group(P<0.05),PD-L1 positive expression and PD-L1 TPS score in micropapillary adenocarcinoma group than in papillary adenocarcinoma group(P<0.05),The positive expression rate of PD-L1 and PD-L1 TPS score in lung cancer tissues of the solid adenocarcinoma group were higher than those in the micropapillary adenocarcinoma group(P<0.05).Conclusion:The expression of PD-L1 varies in different tissue subtypes,and the higher levels of PD-L1 positive expression in tumor tissues of solid lung adenocarcinoma patients may benefit from immunotherapy.
