摘　　要：目的:探究分析乙肝纤维化指标及Child分级与细胞因子7项(IL-2,IL-4,IL-6,IL-10,IL-17A,TNF-α,IFN-γ)之间的关系。方法:选取2021年12月—2022年11月上饶市人民医院收治的乙型肝炎肝硬化住院患者共69例作为本次研究对象,根据Child-Pugh肝功能分级标准将所有患者分成A、B、C三级,采用肝脏瞬时弹性成像(FibroScan)技术检测患者肝脏硬度值(LSM),采用ELISA(酶联免疫吸附法)法以及流式细胞仪检测血清细胞因子水平值,其中包括白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、白细胞介素-17A(IL-17A)、肿瘤坏死因子(TNF-α)和γ干扰素(IFN-γ)7项指标,分析比较各项细胞因子水平值在LSM不同分级以及肝功能Child-Pugh不同级别的差异,探究LSM以及Child-Pugh肝功能分级与细胞因子的相关性。结果:细胞因子IL-2、IL-4、IL-17A、TNF-α、IFN-γ指标在LSM分级F2期、F2-F3期、F3-F4期、F4期对比无明显差异(P>0.05),细胞因子中IL-6,IL-10指标在LSM分级F2期、F2-F3期、F3-F4期、F4期对比具有显著差异,组间差异具有统计学意义(P<0.05),细胞因子IL-6、IL-10水平值与LSM分级呈正相关(P<0.05);细胞因子中IL-2,IL-4,IL-17A,TNF-α,IFN-γ水平值在Child A、Child B、Child C三级间对比差异无统计学意义(P>0.05),细胞因子IL-6,IL-10指标在Child A、Child B、Child C三级间对比具有明显差异,组间差异具有统计学意义(P<0.05),细胞因子IL-6、IL-10水平值与Child分级呈正相关(P<0.05)。结论:乙肝肝硬化患者可能为细胞因子较活跃人群,细胞因子中IL-6,IL-10水平值的动态变化与肝脏硬度情况及Child-Pugh肝功能分级具有一定关联,通过检测细胞因子7项指标可以对肝硬化患者的病情及进展情况进行有效评估。Objective:Explore and analyze the fibrosis indicators of hepatitis B,Child grading and 7 cytokines(IL-2,IL-4,IL-6,IL-10,IL-17A,TNF-α,IFN-γ)The relationship between.Method:A total of 69 hospitalized patients with hepatitis B and cirrhosis admitted to Shangrao People’s Hospital from December 2021 to November 2022 were selected as the research subjects.According to the Child Pugh liver function grading standard,the patients were divided into three levels:A,B,and C.The liver hardness value(LSM)of the patients was measured using transient liver elastography(FibroScan)technology,and the serum cytokine levels were measured using ELISA(enzyme-linked immunosorbent assay)and flow cytometry,This includes interleukin-2(IL-2),interleukin-4(IL-4),interleukin-6(IL-6),interleukin-10(IL-10),interleukin-17A(IL-17A),and tumor necrosis factor(TNF-α)andγInterferon(IFN-γ)Analyze and compare the differences in cytokine levels among different levels of LSM and liver function in different levels of Child Pugh using 7 indicators,and explore the correlation between LSM and Child Pugh liver function grading and cytokines.Result:Cytokines IL-2,IL-4,IL-17A,TNF-α、IFN-γThere was no significant difference in the comparison of indicators in the F2,F2-F3,F3-F4,and F4 stages of LSM grading(P>0.05).The IL-6 and IL-10 indicators in the cytokines showed significant differences in the F2,F2-F3,F3-F4,and F4 stages of LSM grading.The differences between groups were statistically significant(P<0.05),and the levels of IL-6 and IL-10 were positively correlated with LSM grading(P<0.05);IL-2,IL-4,IL-17A,TNF in cytokines-α,IFN-γThere was no statistically significant difference in the levels of IL-6 and IL-10 among the three levels of Child A,Child B,and Child C(P>0.05).There was a significant difference in the levels of IL-6 and IL-10 among the three levels of Child A,Child B,and Child C.The differences between groups were statistically significant(P<0.05),and the levels of IL-6 and IL-10 were positively correlated with the grading of Child(P<0.05
