以梗阻性黄疸为主要表现的转移性胰腺恶性肿瘤4例并文献复习

摘　　要：转移性胰腺恶性肿瘤在临床上非常罕见, 当转移至胰头部时早期易出现黄疸。结合文献复习对本中心4例转移性胰腺恶性肿瘤(1例肾癌、1例乳腺癌、1例子宫平滑肌肉瘤及1例胃癌)的临床表现、诊断要点、治疗选择及组织病理学特点进行分析。4例患者中2例术前可诊断为转移性, 2例无法明确;2例接受了胰十二指肠切除术, 2例接受了姑息性胆肠吻合术。术后患者均未出现黄疸, 至今仍存活。以黄疸为主要表现的转移性胰腺恶性肿瘤患者应尽早手术治疗, 无论是胰十二指肠切除术, 还是姑息性胆肠吻合术, 均可有效延长患者的生存期及生活质量。Metastatic pancreatic metastases are very rare,and pancreatic metastases to the pancreatic head tend to produce jaundice at an early stage.Combined with literature review,the clinical manifestations,diagnostic points,treatment options and histopathological features of 4 cases of metastatic pancreatic malignancies,including a case of kidney cancer,a case of breast cancer,a case of uterine leiomyosarcoma and a case of gastric cancer,were analyzed.Among the 4 metastatic pancreatic malignancies,2 cases were diagnosed as metastatic before surgery,and 2 cases could not be identified.Two patients underwent pancreaticoduodenectomy,while the others underwent palliative bilioenterostomy.None of the patients developed jaundice after surgery,and all were still alive.Patients with metastatic pancreatic malignancies characterized by jaundice should be treated as soon as possible.Both pancreaticoduodenectomy and palliative bilioenterostomy can effectively prolong the survival period and quality of life of patients.
