摘　　要：目的观察胃癌患者癌组织和血清外泌体中LncRNA结肠癌相关转录因子1(CCAT1)、微小核糖核酸-140-3p(miR-140-3p)表达情况并分析其临床意义。方法选取93例行胃癌根治术的胃癌患者为胃癌组,47例胃部良性病变患者为良性病变组,47例健康体检者为健康对照组。采集所有研究对象静脉血,提取血清外泌体,于胃癌患者行胃癌根治术过程中留取患者肿瘤组织样本及癌旁组织样本,采用RT-qPCR法检测血清外泌体及胃癌、癌旁组织中的LncRNA CCAT1、miR-140-3p。对胃癌患者随访5年,记录患者生存时间并计算5年生存率。采用Pearson相关分析法分析患者癌组织及血清外泌体中LncRNA CCAT1与miR-140-3p的相关性;比较不同病理特征胃癌患者癌组织及血清外泌体LncRNA CCAT1、miR-140-3p表达,以胃癌患者癌组织LncRNA CCAT1、miR-140-3p表达的平均值为界限,将患者分为高、低LncRNA CCAT1、miR-140-3p者,采用K-M生存曲线比较高、低LncRNA CCAT1、miR-140-3p患者5年总生存率;采用受试者工作特征(ROC)曲线分析血清外泌体LncRNA CCAT1、miR-140-3p对胃癌的诊断价值。结果患者胃癌组织LncRNA CCAT1表达高于癌旁组织,miR-140-3p表达低于癌旁组织(P均<0.05)。血清外泌体LncRNA CCAT1表达健康对照组<良性病变组<胃癌组,miR-140-3p表达健康对照组>良性病变组>胃癌组(P均<0.05)。Pearson相关分析显示,胃癌患者癌组织及血清外泌体中LncRNA CCAT1表达与miR-140-3p表达均呈负相关(r分别为-0.726、-0.639,P均<0.05)。不同肿瘤分化程度、浸润深度、临床分期及淋巴结转移情况的胃癌患者癌组织及血清外泌体中LncRNA CCAT1、miR-140-3p表达比较差异具有统计学意义(P均<0.05)。癌组织及血清外泌体LncRNA CCAT1高表达的胃癌患者5年总生存率低于LncRNA CCAT1低表达患者,miR-140-3p高表达的胃癌患者5年总生存率高于miR-140-3p低表达患者(P均<0.01)。ROC分析结果显示,血清外泌体LncRNA CCAT1�Objective To observe the expression and clinical significance of long non-coding ribonucleic acid colon cancer associated transcript1(LncRNA CCAT1)and microRNA-140-3p(miR-140-3p)in the cancer tissues and serum exosomes of patients with gastric cancer.Methods Ninety-three gastric cancer patients who underwent radical gastrecto⁃my were selected as the gastric cancer group,while 47 patients with benign gastric lesions were selected as the benign le⁃sion group,and 47 patients with healthy physical examination were selected as the healthy control group during the same period.The expression of LncRNA CCAT1 and miR-140-3p in the gastric cancer tissues,adjacent tissues and serum exosomes of all subjects were detected by reverse transcription quantitative real-time polymerase chain reaction(RT-qPCR).The gastric cancer patients were followed up for 5 years,and we recorded their survival time and the 5-year survival rate.Pearson correlation analysis was used to analyze the correlation between the LncRNA CCAT1 and miR-140-3p in the gas⁃tric cancer tissue and serum exosomes of gastric cancer patients.Based on the average expression of LncRNA CCAT1 and miR-140-3p in the cancer tissue of gastric cancer patients,patients were divided into the high and low LncRNA CCAT1 and miR-140-3p individuals.K-M survival curve was used to analyze the 5-year survival rates of high and low LncRNA CCAT1 and miR-140-3p patients.The diagnostic value of serum exosomes LncRNA CCAT1 and miR-140-3p in the gastric cancer patients were analyzed by receiver operator characteristic(ROC)curve.Results The expression of LncRNA CCAT1 in the gastric cancer tissues of patients was higher than that in the adjacent tissues,and the expression of miR-140-3p was lower than that of the adjacent tissues(both P<0.05).The expression of LncRNA CCAT1 in the serum exosome were as follows:the healthy control group<benign lesion group<gastric cancer group,and the miR-140-3p expression was in the following order:the healthy control group>benign lesion group>gastric canc
